| Objective:Through a randomized controlled study,we observed the effect of Zhang’s Traumatology Manipulation on the clinical efficacy of children with intractable GartlandⅢ supracondylar fracture of humerus in emergency surgery,in order to provide evidence for the application of Zhang’s Traumatology Manipulation in intractable GartlandⅢ supracondylar fracture of humerus in children.Methods:1.A randomized controlled study method was used to carry out a prospective study.The patients with refractory GartlandⅢ supracondylar fracture of the humerus in children who visited Zhangzhou Hospital of Traditional Chinese Medicine from September 2021 to August2022 were selected.According to the time of admission,the patients were divided into the test group and the control group with 30 cases each,according to the random number table method,In the control group(traditional manual reduction+percutaneous Kirschner wire internal fixation),observe the success rate of one reduction,the number of fluoroscopy,and the operation time in the two groups.Through statistical analysis,clarify the effect of Zhang’s orthopedic manual reduction on the operation efficiency of refractory Gartland Ⅲsupracondylar fracture of humerus in children.2.Observe the pain situation in the two groups within 5 days after operation and the movement of the affected limb within 6 months after operation,record the pain score,the elbow flexion and extension range of motion,the lifting angle,and the Flynn standard function evaluation of the elbow joint,and carry out statistical analysis to clarify the effect of Zhang’s Traumatology Technique on the treatment of refractory Gartland Ⅲ supracondylar fracture of the humerus in children.Results:In this study,59 patients were included in the study,including 29 patients in the test group and30 patients in the control group.1.There was no significant difference between the two groups in general data such as age,sex and injury site(P>0.05).2.During the treatment,the one-time reduction rate,operation time and fluoroscopy times of the two groups were significantly better than those of the control group(P<0.05).3.Postoperative Wong-Baker pain score: comparison between groups: the Wong-Baker pain score of the test group was lower than that of the control group on the 1st,2nd,3rd,4th and5 th day after the treatment,and the difference between the other groups was statistically significant(P<0.05).4.Elbow joint function 6 months after operation: the carrying angle and joint flexion and extension activity of the test group were better than those of the control group,with a statistically significant difference(P<0.05).Elbow joint Flynn standard function evaluation:the excellent and good rate of clinical efficacy of the test group was 89.6%,and the total effective rate of the control group was 80.0%.The efficacy of the test group was better than that of the control group,with no statistically significant difference(P>0.05).Conclusions:Zhang’s traumatology manipulation combined with percutaneous Kirschner wire internal fixation for the treatment of refractory Gartland Ⅲ supracondylar fracture of the humerus in children can achieve good clinical results.Compared with the classical traditional method,its operation is more simple,reasonable,standardized and safe.It can not only improve the operation efficiency,effectively alleviate the pain of patients,but also better restore the function of elbow joint,reduce the loss of carrying angle and joint range of motion,to a certain extent avoid the occurrence of cubitus varus deformity and reduce the incidence of complications.Therefore,for the treatment of refractory Gartland Ⅲ supracondylar fracture of the humerus in children,we advocate the use of Zhang’s traumatology technique combined with percutaneous Kirschner wire internal fixation. |
