Comparison Of Manual Reduction With Cast And Internal Fixation With K-wire In The Treatment Of Gartland Type Ⅱ Supracondylar Fracture Of Humerus

Purpose:To investigate the clinical effect of the treatment of the type Ⅱ humeral supracondylar fracture by manual reduction and closed reduction with K-wire.The advantages and disadvantages of the two methods in clinical application were compared to guide clinical work.Methods:30 cases of Gartland Ⅱ supracondylar fracture of humerus treated by manual reduction and30 cases of closed reduction with K-wire were collected from January 2019 to January2020,and age,sex,side,the imaging data of patients were analyzed.The cast removal time,Flynn elbow function score,Baumann angle and complication rate were compared.The clinical effect of manual reduction with cast and closed reduction with K-wire in the treatment of type Ⅱ humeral supracondylar fracture was discussed.Results:All patients were followed up,with an average follow-up of 10.6 months(6-20 months),and an average age of 3.75 years(1-9 years).Among them,31 males,29 females,33 left and 27 on the right.There was no significant difference in the cast removal time(27.47 ±2.15 vs 27.57 ± 2.08)days,the incidence of complications(0.00% vs 3.33%)and Baumann’s angle(70.88 ° ± 5.36 ° vs 71.56 ° ± 4.36 °)in the manipulation group and the K-wire group(P > 0.05).One case of ulnar nerve injury was found in the K-wire group,and no postoperative complications were found in the manipulation group.In the clinical efficacy evaluation,the results of Flynn elbow function score in final follow-up indicated that the excellent rate of manipulation group and K-wire group were 100.00%,and 28 cases were excellent,2 were good,0 were middle and 0 were poor;30 cases were excellent,0 were good,0 were poor in the group.There was no statistical significance between the two groups(P > 0.05).Conclusion:The traditional Chinese manual reduction combined with cast has the same effect as that of the internal fixation by K-wire on the treatment of type Ⅱ supracondylar fracture of humerus.The traditional Chinese manual reduction has the advantages of less complications,less pain,shorter treatment course and lower cost,which is worth further promotion in clinical practice.
