Clinical Observation Of Bushen Huoxue Decoction On Swelling,Pain And Functional Activity After Arthroscopic Rotator Cuff Repair

Objective: To observe the clinical effect of Bushen Huoxue decoction on swelling,pain,functional activity and blood flow around rotator cuff after arthroscopic rotator cuff repair(kidney deficiency and blood stasis).Methods: Sixty patients with rotator cuff injury diagnosed by the Department of Sports Medicine of the second people’s Hospital affiliated to Fujian University of traditional Chinese Medicine from January 2022 to January 2023 were randomly divided into control group(n =30)and experimental group(n = 30).The control group was routinely given basic treatment(ice compress,intravenous drip of ibuprofen injection and rehabilitation exercise),while the experimental group was given basic treatment plus oral Bushen Huoxue decoction.The observation indexes included quantitative scores of TCM symptoms and signs,swelling evaluation,pain evaluation,passive range of motion evaluation of shoulder joint(flexion,abduction,pronation,internal rotation)and blood flow around rotator cuff.The indexes of patients were recorded before operation,1 day,1 week,2 weeks and 4 weeks after operation.Results:1.There was no significant difference in age,sex,limb type,BMI and course of disease between the two groups(P > 0.05).There was no significant difference in VAS pain score and shoulder range of motion(flexion,abduction,external rotation,internal rotation)between the two groups before operation.It is comparable.2.There was no significant difference in TCM syndrome scores between the two groups before operation and 1 day after operation(P > 0.05).After operation,the TCM syndrome scores of the two groups decreased significantly with the passage of time,and there was significant difference between the control group and the experimental group at 1 week,2weeks and 4 weeks after operation(P < 0.05).The curative effect of traditional Chinese medicine showed that the control group was cured in 0 cases,significant in 0 cases,effective in 14 cases and ineffective in 16 cases,while in the experimental group,it was cured in 0cases,significant in 0 cases,effective in 26 cases and ineffective in 4 cases.There was significant difference between the two groups(P < 0.05).The TCM syndrome of the experimental group was significantly better than that of the control group.3.There was significant difference in VAS pain score between the control group and the experimental group at 1 week,2 weeks and 4 weeks after operation.The pain relief in the test group was more obvious than that in the control group.4.There was significant difference in the increase of limb swelling circumference between the control group and the experimental group at 1 week,2 weeks and 4 weeks after operation(P <0.05).The swelling in the test group was significantly better than that in the control group.5.The range of motion of shoulder flexion,abduction,external rotation and internal rotation of the control group and the experimental group were significantly improved with the passage of time,and there were significant differences among the three groups(P < 0.05).There were significant differences in shoulder flexion,abduction and external rotation between the two groups at 1 week,2 weeks and 4 weeks after operation(P < 0.05).There was significant difference in shoulder internal rotation between the two groups at 2 weeks and 4 weeks after operation(P < 0.05).The range of motion of shoulder joint in the test group was significantly better than that in the control group.6.There was no significant difference in the score of peri-rotator cuff vessels between the two groups 4 weeks after operation(P > 0.05).7.During the period of medication,there were no adverse reactions,and the liver and kidney function was not obviously abnormal,so it was safe.Conclusion: For patients with arthroscopic rotator cuff repair(kidney deficiency and blood stasis type),Bushen Huoxue decoction can reduce postoperative pain,relieve the degree of swelling,improve the functional range of motion of shoulder joint,but has no significant advantage in promoting blood flow around rotator cuff.
