Diagnostic Value Of Plasma NT-proBNP For Heart Failure In Patients With CKD Without Dialysis

Objective : Explore chronic kidney disease at stage CKD1-5(chronic kidney disease,CKD)combination of heart failure patients without dialysis(heart failure,HF)N end of plasma B-type natriuretic peptide original(N-terminal probrain natriuretic peptide,NT-pro BNP)the critical value,so as to provide reference for clinical diagnosis,early recognition and intervention of HF progress,reduce the non dialysis patients with CKD hospitalization rate and mortality.Methods : Collected through retrospective study,2021.12-2022.3 months in yunnan province,the first people’s hospital kidney CKD1-5 period of dialysis patients in 200 cases as the research object,126 cases of male,female 74 cases,including 68 cases of patients with heart failure,132 cases of patients without heart failure.Spearman analysis plasma NT-pro BNP and left ventricular end-diastolic volume,ejection fraction,blood pressure,age,glomerular filtration rate,such as correlation,the receiver-operating characteristic curve(receiver operating characteristic curve,the ROC curve)analysis of the NT-pro BNP the critical value in the diagnosis of HF.Results:Select 95% confidence interval observation area under the ROC curve(area under concentratio n-time curve,AUC): CKD1-2(AUC = 1,P < 0.021,1sensitivity and degree 1);CKD3(AUC = 0.804,P < 0.002,0.75 sensitivity and degree of 0.95);CKD4(AUC = 0.862,P < 0.001,0.8 sensitivity and degree of 0.931);CKD5(AUC = 0.924,P < 0.00 01,sensitivity of 0.909 and 0.868).CKD1-phase 2 NT-pro BNP threshold of 56 patients without dialysis 7 pg/ml,CKD3-5 issue of 783 pg/ml,respectively 1863 pg/ml,2072.5 pg/ml.Plasma NT-pro BNP level and left ventricular end-diastolic volume,systolic blood pressure,serum creatinine,urinary inhibition C,blood urea nitrogen,serum muscle calcium egg white I,parathyroid hormone,phosphorus,potassium ions was positively linear correlation.Plasma level of NT-pro BNP patients with glomerular filtration rate,ejection fraction,hemoglobin,serum albumin,calcium ion,platelets are negatively related to the line.Plasma NT-pro BNP levels with age,sex,diastolic blood pressure,heart rate,total cholesterol,chloride ion,sodium ion,uric acid has no relevance.Conclusions : The optimal threshold value of plasma NT-pro BNP for HF diagnosis in non-dialysis patients at stage CKD1-5:1.in CKD1-2 stage 567pg/ml,2.in CKD3 stage 783pg/ml,3.in CKD4 stage 1863pg/ml,4.in CKD5 stage 2072.5pg/ml,Above,when NT-pro BNP reaches a critical value,it suggests that HF may occur in non-dialysis patients with CKD,and it is necessary to be vigilant and take intervention measures when necessary.
