Correlational Study On Type 2 Diabetic Kidney Disease Patients With Dyslipidemia And TCM Syndromes

Objective: To observe the TCM syndrome types of patients with type 2 diabetic kidney disease complicated with dyslipidemia,and to study the differences of general conditions and physical and chemical indexes among patients with different syndrome types,so as to provide certain ideas for the treatment of type 2 diabetic kidney disease complicated with dyslipidemia.Methods: A total of 229 patients with type 2 diabetic kidney disease complicated with dyslipidemia were collected from December 2021 to December 2022 in the Department of Endocrinology and Metabolism,Affiliated Hospital of Changchun University of Chinese Medicine.Clinical data of patients,including general data,information of four TCM diagnoses and related clinical laboratory indicators,were collected using the case collection table.Excel 2019 software was used to establish the database for data integration,and SPSS 25.0 statistical software was used for data statistics.Results:1.The distribution of TCM syndrome types: the syndrome of liver and kidney Yin deficiency accounted for the highest proportion in this syndrome,75 people,accounting for32.8% of the total number,followed by the syndrome of qi and Yin deficiency in 70 people,Yin and Yang deficiency in 53 people,and spleen and kidney Yang deficiency in 31 people.Phlegm-dampness syndrome accounted for the largest proportion in 74 patients,accounting for 32.3% of the total patients,followed by blood stasis syndrome 67 patients,damp-heat syndrome 51 patients and no concurrent syndrome 37 patients.2.General information: There were statistical differences between TCM syndrome types and patients’ age,course of disease,DKD stage,BMI,history of coronary heart disease,history of hypertension,smoking history and drinking history.The patients with Qi-Yin deficiency syndrome were younger in age and shorter in course of disease.The DKD stage was mainly microalbuminuria stage.The patients with yin-yang deficiency syndrome were older,the course of disease was longer,and the DKD stage was mainly clinical albuminuria stage.Patients with no concurrent syndrome were younger in age,shorter in course of disease and lower in BMI.Patients with phlegm-dampness syndrome are older,with longer course of disease and larger BMI.There were no significant differences in gender,waist-to-hip ratio,blood pressure and family history of DM among all syndrome types.3.Physical and chemical indexes: there were statistical differences in LDL-C,HDL-C,Apo-A1,LPa,e GFR and UACR among different syndrome types.The levels of LDL-C,HDL-C and Apo-A1 were lower in dampness-heat syndrome,LPa level was lower in Qi and Yin deficiency syndrome,e GFR level was lower in Yin and Yang deficiency syndrome,and UACR level was higher.There were no significant differences in the levels of TC,TG,Apo-B,FPG,HGB and UA among the syndrome types.4.Types of dyslipidemia were mainly hyper TGemia(103 persons,accounting for45.0%),followed by low HDL-C emia,mixed hyperlipidemia and hypercholesterolemia.Conclusion:1.The most common TCM syndromes in patients with type 2 diabetic kidney disease complicated with dyslipidemia are liver-kidney Yin deficiency syndrome,and most patients are accompanied by concurrent syndromes,mainly phlegm-dampness syndrome.2.The most common type of dyslipidemia in patients with type 2 diabetic kidney disease complicated with dyslipidemia is hyper TGemia.In hyper TGemia,the TCM syndromes are mainly Qi and Yin deficiency syndrome,and the concurrent syndromes are mainly phlegm-dampness syndrome.3.Age,course of disease,DKD stage,BMI,smoking history,drinking history,coronary heart disease history and hypertension history were different among different TCM syndrome types,and LDL-C,Apo-A1,FPG,e GFR,UACR and other physical and chemical examination indexes were also different among different syndrome types.4.Gender,DKD stage,family history of DM,smoking history,and history of hypertension were different among different types of dyslipidemia.There were no significant differences in various indexes of blood glucose and renal function among different types of dyslipidemia,but there was a certain correlation.
