Objective: To observe the clinical efficacy and safety of Tiao Qi Zhi Tong Acupuncture combined with bloodletting therapy in the treatment of Tension Type Headache and to make an objective evaluation. Methods: Using the SPSS random number table method, the 65 cases of tension-type headache collected were randomly divided into 33 cases in the treatment group(shedding 1case)and 32 cases in the control group according to the order of admission. In the treatment group, the Tiao Qi Zhi Tong formula was used, which includes Lieque point,Fenglong point and Ligou point.Baihui point,Shenting point,Sishencong point,Fengchi point and Touwei point are also the main acupuncture points for treatment which were combined with bloodletting of the Taiyang point. In the control group, the main acupuncture points of Baihui point,Shenting point,Sishencong point,Fengchi point,Touwei point and Taiyang point were used, while the distal points of the two groups were selected according to the meridian dialectic.Acupuncture treatment was carried out 3 times a week and bloodletting 2 times a week for a period of 4 weeks. The degree of improvement in clinical symptoms and the visual analogue scale(VAS),the Headache Impact Test Scale-6(HIT-6)and the Self-Rating Anxiety Scale(SAS)were evaluated and compared in both groups before and after treatment and during the follow-up period to assess the therapeutic effects of the two treatments.Results:1.Main efficacy index: After treatment, the significant improvement rate of headache in the treatment group was 90.63%,higher than 68.75% in the control group(P < 0.05).At the follow-up period, the significant improvement rate of headache was 84.38% in the treatment group and 59.38% in the control group(P < 0.05),with better long-term efficacy in the treatment group.2.Secondary efficacy indicators:(1)Frequency of headache attacks: After treatment, the number of headache days decreased by 8.94±2.93 days in the treatment group and 7.31±2.53 days in the control group,P<0.05.During the follow-up period, the number of headache days decreased by 8.81±3.04 days in the treatment group and 7.00±2.21 days in the control group,P<0.05.(2)Headache duration score: After treatment, the headache duration score decreased by0.96±0.62 in the treatment group and 0.59±0.47 in the control group,P<0.01.During the follow-up period, the headache duration score decreased by 1.01±0.54 in the treatment group and 0.64±0.48 in the control group,P<0.01.(3)VAS score: After treatment, the VAS score decreased by 3.41±0.80 in the treatment group and 2.37±1.01 in the control group,P<0.01.During the follow-up period, the VAS score decreased by 3.47±0.72 in the treatment group and 2.44±1.25 in the control group,P<0.01.(4)HIT-6 score: After treatment, the score was reduced by 13.59±6.66 in the treatment group and 10.22±5.17 in the control group,P<0.05.During the follow-up period, the score was reduced by 14.63±6.41 in the treatment group and 10.38±5.20 in the control group,P<0.05.(5)SAS score: After treatment, the score was reduced by 12.31±5.58 in the treatment group and 7.84±5.06 in the control group,P<0.01.During the follow-up period, the score was reduced by 12.13±5.90 in the treatment group and 7.09±5.13 in the control group,P<0.01.Conclusion: The combination of Tiao Qi Zhi Tong acupuncture with bloodletting for the treatment of tension-type headache can effectively relieve the pain symptoms of patients, reduce the frequency of headache attacks, and have a long-term effect. This treatment method is better than that of the control group, especially in improving headache symptoms(attack frequency, duration and pain score). The method is economical and safe, and no adverse effects were observed during the treatment period,so it is worthy of clinical application.
