Objective:Relevant surveys have found that the incidence,prevalence,mortality and diseaseburden of strokein permanent residents in Jiangxi Province are all above the nationalaverage level,especially theincidence and prevalence ofstroke inpeople aged 40 years and above increased rapidly,suggesting that researchers should pay attention totheprevention and control of stroke in Jiangxi Province.As a kind ofcomplex diseasewith the interaction of genetic and environmental factors,stroke needs researchersto conduct a comprehensivestudy on the interaction between different genetic and environmental factors to better understand the genetic mechanism ofstroke.Therefore,the purposeof this study IS toinvestigatethe general demographic data,behavioral lifestyledata,diet,related biochemical indicators of patientswith Ischemicstroke(IS)in southern Jiangxi,and to analyze the environmental risk factors of stroke insouthern Jiangxi.To explore theinteraction between ALOX5 AP and SOCS3,as well as the interaction between genes and environmental factors,on IS patients in southern Jiangxi.Method:This study was conducted in Southern Jiangxi Province using a 1:1 matched case-control method.Atotal of 114 patients with IS were collected and 114 matchedhealthy controls who wereof thesame gender and withan agedifference of≤2 years wereselected according totheir age.Physical examinations,questionnairesurveys and related biochemical indicators were performed.Screening IS sensitive genes and their sites using meta-analysis and NCBI databases.Conditional Logisticregression wasused toanalyze the roleof polymorphism and environmental factors inthe occurrenceof IS,andmulti-factor dimension reduction method(MDR)was used to explorethe interaction between gene-gene and gene-environmentandits effecton IS.Result:The environmental risk factors for ischemic stroke inthepopulation of Gannan area include:IS family history,hypertension,diabetes,central obesity,highdensity lipoproteincholesterol reduction,smoking,poorsleep quality and often psychological stress.Regular physical activity(including vigorous physical activity,moderate physical activity,and walking),lower total cholesterol,andhigher LDLcholesterol levels wereprotectivefactors for IS.According tothe analysis of the food situation,salty taste,spicy IS a risk factor;In terms ofdiet,salty and spicy tastewere the riskfactors forischemic strokein Gannan area.The main food intake≥300g/ day,white meat intake≥42.87g/ day,salted vegetable intake≥0.72g/ day and cured meat sausage intake≥0.43g/ daywere therisk factors of IS.Theintake of dairy products≥35.72g/day,fruit≥100g/ dayand vegetables≥200g/ daymight be theprotectivefactors for ischemic strokein Gannanarea.There IS also an interaction between food tasteand fasting glucoselevels(OR=27.39,95%CI=2.66-282.31,P=0.0015)that both affect the development of IS.The SOCS3 gene rs9914220 was associated with the occurrence of IS inthe population of Southern Jiangxi Province.In therecessive model,compared with theindividual carrying the TTgenotype,the individual carrying CC+TC had a reduced risk of IS(OR=0.429,95% CI:0.185-0.993,P = 0.044).In theallele model,individuals with the Tallelehad an increased risk ofdeveloping IS compared with individuals with the C allele(OR=6.243,95% CI:3.837-10.158,P < 0.001).Therewas a linkagedisequilibrium between rs9914220 and rs8064821;Haplotype analysis revealed that SOCS3 genehaplotype A-C-C might be a risk factor for IS(OR=4.536,95% CI: 2.000-10.287,P < 0.001).Haplotype A-T-Cmay be a protective factor for IS(OR=0.490,95% CI: 0.312-0.770,P=0.002).The genotypes,genetic models,and IS risk at rs4969168 inthe SOCS3 gene,rs8064821,and rs9579646 in the ALOX5 AP genewere not statistically significant(P>0.05).Interaction between SOCS3 gene and ALOX5 AP gene andenvironment.Conclusion:Family history of ischemic stroke,hypertension,diabetes,central obesity,elevated high density lipoprotein cholesterol,smoking,poor sleep quality and often psychological stress are theenvironmental risk factors for theoccurrence of ischemic strokeinpeople in Gannan area.Regularphysical activity(including vigorous physical activity,moderatephysical activity and walking activity),lower total cholesterol level and higherlowdensity lipoprotein cholesterol level arethe protective factors of ischemic stroke.In terms of diet,salty and spicytaste werethe risk factors for ischemic stroke in Gannan area.Themain food intake≥300g/ day,whitemeat intake≥42.87g/ day,salted vegetable intake≥0.72g/ day and cured meat sausage intake≥0.43g/day werethe risk factors of IS.The intake ofdairy products≥35.72g/ day,fruit≥100g/dayand vegetables≥200g/ day might be the protective factors for ischemic strokein Gannan area.The T allele of SOCS3 gene rs9914220 is associated with the increased riskof ischemic strokein Gannanpopulation.No association was found between SOCS3 gene rs4969168,rs8064821 locus and ALOX5 AP geners9579646 locus polymorphism and ischemic strokein Gannanarea.Thereis gene-gene interaction,SOCS3 gene haplotype A-C-C increases the risk ofischemic stroke in Gannan area,while haplotype A-T-Creduces the risk of ischemic stroke.No interaction between gene and environment was found inpatients with ischemic stroke in Gannanarea.
