| Objective The aim of this research is to explicit the relationship between Janus kinase 2signal transducers(JAK2)/ activators of transcription3(STAT3)signal path and pathogenesis、stages of disease 、 order of severity in acute pancreatitis(AP),explicit the influence of probiotic on JAK2/STAT3 signal path and blood lipid,to prevent and guide the treatment of HLAP.Methods All patients’ samples were collected from People’s Hospital of Inner Mongolia Region that were diagnosed with AP from August 2020 to December 2022.Rely on the etiological factors of AP,splitted into hyperlipidemic acute pancreatitis(HLAP)and non hyperlipidemic acute pancreatitis(NHLAP).Rely on the classification of AP,splitted into mild acute pancreatitis(MAP),moderate severe acute pancreatitis(MSAP)and severe acute pancreatitis(SAP).Collect serum samples of patients,using enzyme-linked immunosorbent assay(ELISA)measure the levels of JAK2、phospho-janus kinase 2(P-JAK2)、STAT3、phospho-signal transducers and activators of transcription3(P-STAT3).Using Epidata database enter the clinical and experimental data.Compare the levels of JAK2、P-JAK2、STAT3、P-STAT3 of HLAP and NHLAP groups,and analyse the relationship between JAK2、P-JAK2、STAT3、P-STAT3 and pathogenesis、 stages of disease、order of severity of AP in SPSS 25.0 software.Consenting to inclusion group was given compound probiotics 2g /day,on the basis of routine treatment for a month.The control group was given the routine treatment with the same length of hospitalization and the same order of severity,the level of JAK2、P-JAK2、STAT3、P-STAT3 and blood lipid were compared between the two groups.Results The average age of HLAP group was younger than NHLAP group(40.60±8.46 years old,P=0.000),and the proportion of males was significantly higher than HLAP group(74.2%,P=0.040);the prevalence rate of fatty liver(50.0%)and diabetes(33.9%)were significantly higher in the HLAP group(P=0.005、0.023);the rate of smoker(59.7%)and drinker(46.8%)were higher in the HLAP group(P=0.012、0.009).The levels of aspartate transferase(AST)、alanine transferase(ALT)、glutamyl transferase(GGT)、total bilirubin(TBIL)、direct bilirubin(DBIL)in HLAP group were lower than NHLAP group(P=0.000、0.003、0.012、0.002、0.002);the levels of blood amylase,pancreatic amylase and lipase in HLAP group were lower than NHLAP group(P=0.019、0.002、0.048).As disease’s condition improving,the expression of JAK2、P-JAK2、STAT3、P-STAT3 were decreased in both HLAP and NHLAP,with more significant degree in HLAP have statistical significance(P<0.05);the hospital admission expression of JAK2 、 STAT3 were more higher in HLAP group than NHLAP,having statistical significance(P=0.026、0.006).Compared with MAP,the expression of JAK2、P-JAK2、STAT3、P-STAT3 were increased in MSAP and SAP in both of HLAP and NHLAP groups,having statistical significance(P<0.05);compared with NHLAP group,the expression of JAK2、P-JAK2、STAT3、P-STAT3 were increased in MAP、MSAP and SAP of HLAP group,especially in MAP,the difference was statistically significant(P=0.004、0.012、0.000、0.004),having no statistical significance in MSAP and SAP.After using compound probiotics for a month,compared with the control group,the level of JAK2、P-JAK2、STAT3、P-STAT3 were decreased in treatment group(P<0.05),the blood lipid level of treatment group is lower than the control group,especially the difference of low density lipoprotein was statistically significant(P=0.006).Conclusion During occurrence and development process in acute pancreatitis,multiple inflammatory mediators aggravate the damage of various organs and dysfunction through JAK2/STAT3 signal path,even lead to systemic inflammatory response syndrome(SIRS).In early inflammatory stage,JAK2/STAT3 signal path fall into waterfall conduction,especially in severe case,the inflammatory response of HLAP is more intensely than NHLAP.Probiotics can regulate intestinal microecology,decrease the level of inflammatory,regulate lipid metabolism in HLAP. |
