| ObjectiveThis study mainly observed the effect of postherpetic neuralgia treat by thermal moxibustion,summary of the distribution pattern of thermal points and its effect on serum IL-6 level,and preliminarily summarized the distribution of heat-sensitive points in PHN patients,so as to seek new methods and new ideas for the treatment of PHN patients.MethodsIn our study,a total of 77 patients with PHN who met the criteria established by us were selected and randomly divided into two groups.One was treated with heat-sensitive moxibustion combined with Buyang Huanwu decoction orally,and the another one was treated with specific electromagnetic spectrum(TDP magic lamp)plus oral pregabin.After four weeks of treatment,we evaluated the changes of VAS score,IL-6 level,and quality of life score(DLQI score)of the two groups before and after treatment,and summarized the obtained heat-sensitive acupoints,and initially concluded the distribution law of heat-sensitive points of PHN.Results1.Baseline: Affected by objective factors,we finally included 77 eligible PHN patients,38 in the treatment group and 39 in the another one.The age,gender,VAS score,course of disease,DLQI score and serum IL-6 level of the two groups were compared with P > 0.05,which could be analyzed and compared.2.VAS scores: After 4 weeks of treatment,we found that the improvement of VAS score in the treatment group was more obvious than that in the control group,which means that heat-sensitive moxibustion combined with Buyang Huanwu decoction can improve the pain symptoms of PHN patients more obviously.3.DLQI score:After 4 weeks of treatment,we found that the decrease of DLQI score in the treatment group was greater than that in the another one(P<0.05),suggesting that heat-sensitive moxibustion combined with Buyang Huanwu decoction had a more significant effect than specific electromagnetic spectrum(TDP magic lamp)plus oral pregabalin.4.Serum IL-6 levels: The mean values of serum IL-6 before treatment were,respectively,(236.73±68.71)ng/L in the treatment group and(227.21±59.01)ng/L in the another one(P>0.05);the mean values of serum IL-6 after treatment were,respectively,(87.06±34.62)ng/L in the treatment group and(118.61±44.54)ng/L in the another one.(both P< 0.05),indicating that both treatments had an inhibitory effect on serum IL-6.After treatment,the post-treatment comparison was more effective in the treatment group than the another one(P< 0.05),indicating that the therapy using thermal moxibustion combined with Buyang Huanwu Soup was more effective in decreasing serum IL-6 in patients with PHN.5.Clinical efficacy: In the treatment group treated with heat-sensitive moxibustion,the total effective rate was 96.67% after 4weeks of treatment.Another one was irradiated by the magic lamp,we calculated a total effective rate of 86.67%.This indicates that heat-sensitive moxibustion combined with Buyang Huanwu decoction is more effective for PHN patients.6.Adverse reactions: One patient in the treatment group was scalded during heat-sensitive moxibustion,and no obvious discomfort was felt after applying burn moisturizing cream.In the control group,2 patients had slight abnormal platelet count and considered adverse reactions of pregabalin.The dyslipidemia was not aggravated one week after drug withdrawal,and other abnormalities were not observed..7.Comparison of recurrence rate: The recurrence rates of the two groups were 6.1% and 29.2%,respectively(P<0.05),indicating that the recurrence rate of PHN patients after the treatment of heat-sensitive moxibustion combined with Buyang Huanwu decoction was smaller.8.Distribution pattern of heat-sensitive points: Among all patients,the meridians with a high frequency of heat-sensitive points were mainly the Governor’s Vessel,Ren Vessel,Hand Shao Yin Heart Meridian and Foot Convulsive Yin Liver Meridian,and the A-Yi point,which is the site of the original disease."The majority of patients can have heat sensitization reactions such as heat transmission,heat expansion,and so on,with a compound moxibustion sensation,and some patients have a single moxibustion sensation of an itchy sensation at the affected area;Lumbar Yangguan is the site with the highest incidence of heat sensitization reactions except for the A-Yi point,where the heat sensitization reactions are all compound moxibustion sensations,with heat transmission,heat transmission,and heat expansion.The majority of patients had all three sensations simultaneously,and two patients had non-thermal sensations such as itching and numbness.Conclusion1.The higher the serum IL-6 level,the higher the pain degree of PHN seems to be.Therefore,we initially concluded that heat-sensitive moxibustion combined with Buyang Huanwu decoction in the treatment of PHN patients may ultimately achieve the treatment goal by reducing the activation or secretion of inflammatory factors in the serum IL-6 level of patients.2.The combination of thermal moxibustion and tonic Yang Returning Five Soup was effective in treating patients with PHN,reducing serum IL-6levels in PHN patients,significantly reducing their pain symptoms and improving their quality of life.This protocol is safe,has few side effects,is simple to operate and is cost effective,and is worthy of further promotion.3.In PHN patients,thermal sensitization of acupuncture points has a clear distribution pattern,mostly on the trunk and head and neck,mostly in specific areas such as A-Yi,Liao-Yang-Guan,Ji-Quan,Shen-Que,Shi-Zhu-Kong and Cataract.When using thermal moxibustion as the main treatment for this group of patients,we can give priority to exploring the above-mentioned high incidence points. |
