Objective:To investigate the relationship between triglyceride-glucose index(TyG)and major adverse cardiovascular events(MACE)in patients with heart failure after acute myocardial infarction.Methods:From October 2020 to October 2021,the clinical and laboratory data of 122 patients with acute myocardial infarction and post-infarction heart failure diagnosed in the Second Affiliated Hospital of Shanxi Medical University were retrospectively analyzed,and the TyG index was calculated.TyG median was divided into high TyG group and low TyG group.Spearman correlation analysis was used to evaluate the correlation between TyG index and cardiovascular risk factors.The model was established using COX risk regression analysis for univariate and multivariate analysis,and ROC curve was used to evaluate the predictive performance of TyG index.Results:(1)During the 12-month follow-up,38(31.35%)of the 122 patients with heart failure after acute myocardial infarction had MACE events.In the high TyG index group,the incidence of MACE was significantly increased.(2)TyG index is positively correlated with body mass index(BMI),fasting blood glucose(FBG),total cholesterol(TC),triglyceride(TG),low-density lipoprotein(LDL)and other indicators,while with heart rate(HR),High-density lipoprotein(HDL)was negatively correlated.(3)The COX risk regression analysis adjusted by various risk factors showed that TyG index was positively correlated with the incidence of MACE events,HR was 2.277(95%CI: 1.484-3.494,P<0.001).(4)According to the Kaplan-Meier survival analysis curve,the survival rate of patients in the high TyG index group was lower than that in the low TyG group(P<0.05).(5)After adjusting for age,gender,BMI,hypertension,smoking history,heart rate(HR),high-density lipoprotein(HDL),low-density lipoprotein(LDL),creatinine(Cr),and ejection fraction(LVEF)After other confounding factors,the TyG index could still predict the occurrence of MACE events(P<0.05).(6)The area under the ROC curve of the TyG index for predicting MACE events was 0.707,the sensitivity was 63.2%,the specificity was 82.1%,and the best cut-off point was 9.23.Conclusions:TyG index is associated with the prognosis of patients with heart failure after acute myocardial infarction,and TyG index is a predictor of MACE events in patients with heart failure after acute myocardial infarction.
