Objective:CD5 antigen-like protein(CD5L)and cluster of differentiation 36(CD36)are both involved in the development of renal diseases. To find out if CD5 L and CD36 were associated with clinical and pathological indicators of patients with Ig A nephropathy(Ig AN),we did some research. Methods:In this study, kidney tissues, serum, and urine samples were collected from 45 patients with Ig AN who had been diagnosed at the Third Hospital of Shanxi Medical University between July 2022 and December 2022.Normal kidney tissues from 20 patients who had undergone nephrectomy and serum and urine samples from 20 healthy volunteers were chosen as controls.Immunohistochemistry(IHC)was used to measure the expression levels of CD5 L and CD36 in renal tissues.Enzyme-linked immunosorbent assay(ELISA)was used to measure the concentrations of CD5 L and CD36 in serum and urine. In order to investigate the relationship between CD5 L,CD36,and renal indicators in Ig AN patients,Spearman’s rank correlation analysis was used. The diagnostic potency of CD5 L and CD36 for Ig AN was assessed using a receiver operator characteristic(ROC)curve. Results:Compared with the control group, the levels of CD5 L and CD36 of Ig AN patients were significantly higher in the renal tissue and urine(P < 0.001),while there was no difference in serum CD5 L and CD36(P > 0.05).Spearman’s rank correlation analysis showed that Urinary CD5 L levels in patients with Ig AN were not only positively correlated with 24 h urine protein, serum creatinine(Scr),Lee’s classification,T,and chronicity index(r=0.622,0.350,0.449,0.457,0.735,P<0.05)but also negatively correlated with estimated glomerular filtration rate(e GFR)(r=-0.572,P<0.001).Urinary CD36 levels in patients with Ig AN were positively correlated with 24 h urine protein,Scr,Lee’s grade,T,and chronicity index(r=0.590,0.410,0.394,0.508,0.780,P<0.05)and negatively correlated with e GFR(r=-0.557,P<0.001).According to the receiver operating characteristic curve results, the areas under the curve for the combination diagnosis of Ig AN using urinary CD5 L and CD36 were 0.796,0.768 and 0.857,respectively(P < 0.05).Conclusions:CD5L and CD36 play an important role in the pathogenesis of Ig AN.Urinary CD5 L and CD36 levels can potentially reflect clinical and pathological severity and may represent a useful biomarker in Ig AN.
