The Clinical Efficacy Of Loop Plate Versus Clavicular Hook Plate In The Treatment Of Neer Type Ⅱ Distal Clavicle Fractures:a Meta-analysis

Objective:To compare the clinical efficacy of loop plate and clavicular hook plate in the treatment of Neer type Ⅱ distal clavicle fractures.Methods:Randomized and non-randomized controlled studies on loop plate versus clavicular hook plate in the treatment of Neer type Ⅱ distal clavicle fractures were searched from CNKI,VIP,Wanfang,Pubmed,Embase,the Cochrane Library and Web of Science.The retrieval period is from self-built database to December 2022.Literature screening,data extraction and literature quality evaluation were conducted by two researchers respectively,and meta-analysis of observational indicators was performed using Revman5.1 software.Results:A total of 14 literatures were included,including 772 patients with Neer type Ⅱ distal clavicle fractures.The results of meta-analysis showed that:(1)In terms of fracture healing,the two treatments showed similar performance,with no statistical significance in mean fracture healing time and delayed fracture healing rate.(2)Shoulder joint function: loop plate group had higher CMS score,ASES score and lower VAS score at the last follow-up;(3)The loop plate group had a lower incidence of secondary surgery;(4)Postoperative complications: the loop plate group had less total postoperative complications,but there was no statistical difference between the two groups in terms of clavicle refracture and internal fixation loosening.Conclusion:In this study,the loop plate has the same performance as the clavicular hook plate in fracture healing,but it has certain advantages in reducing postoperative complications,improving shoulder joint function,reducing postoperative pain,and reducing the incidence of secondary surgery.It is a feasible choice for the treatment of distal clavicle fracture surgery.This conclusion still needs to be further verified by high-quality,large sample,multi-center randomized controlled studies.
