| Objective:To investigate the value of mean platelet volume-to-lymphocyte ratio(MPVLR)and mean platelet volume-to-platelet count ratio(MPVPCR)in the auxiliary diagnosis of persistent pulmonary hypertension of the newborn(PPHN)in moderate and late preterm infants.Methods:A total of 100 moderate and late preterm infants with respiratory failure admitted to the Department of Neonatology and Neonatal Intensive Care Unit of Shanxi Children’s Hospital from January 1,2019 to December 31,2021 were retrospectively collected.According to whether the infants were complicated with persistent pulmonary hypertension of the newborn,they were divided into PPHN group and control group.The two groups were compared in terms of general baseline data and laboratory data(Blood samples were collected within 2 hours after birth).Spearman correlation analysis was used to evaluate the correlation between laboratory data and PPHN.Multivariate Logistic regression analysis was used to evaluate the independent predictors of PPHN.ROC curve was used to analyze the optimal cut-off points of MPVLR and MPVPCR for the diagnosis of PPHN in moderate and late preterm infants.Results:There were significant differences in gestational age,birth weight and delivery mode between the PPHN group and the control group(P < 0.05).The neutrophil(NE)and mean platelet volume(MPV)of the PPHN group were higher than those of the control group,and the lymphocyte(LYM)and platelet(PLT)were lower than those of the control group,and the differences were statistically significant(P < 0.05).The levels of neutrophil-to-lymphocyte ratio(NLR)[2.14(0.96,3.55)],MPVLR [2.85(1.84,3.92)]and MPVPCR(0.042±0.012)in PPHN group were higher than those in control group【[1.32(0.66,1.91)],[2.20(1.66,2.64)],(0.034±0.007)]】,and the differences were statistically significant(P < 0.05).NE,MPV,NLR,MPVLR and MPVPCR were positively correlated with PPHN(r = 0.252,0.354,0.281,0.276,0.334,P < 0.05),LYM and PLT were negatively correlated with PPHN(r =-0.221,-0.229,P < 0.05).Logistic multivariate regression analysis showed that cesarean delivery and MPVPCR could be used as independent predictors of PPHN(OR = 7.559,1.848,P < 0.05).The cut-off values of MPVLR and MPVPCR for diagnosing PPHN were 2.64 and 0.042,respectively.Conclusion:The levels of MPVLR and MPVPCR in the PPHN group are higher than those in the control group,which can be used as laboratory indicators for auxiliary diagnosis of PPHN.Cesarean section and MPVPCR can be used as independent predictors of PPHN.Due to the limitations of this study,the value of both in the severity and prognosis of children with PPHN has not been confirmed,and further research is needed. |
