Clinical Observation Of Wuwei Han Dynasty Medical Jian Yufang In The Treatment Of Qi Stagnation And Blood Stasis Knee Osteoarthritis

Objective:To observe the clinical efficacy and evaluate the safety of Wuwei Han Dynasty Medical Jian Yufang in the treatment of knee osteoarthritis of qi stagnation and blood stasis type,in order to provide theoretical basis for Wuwei Han Dynasty Medical Jian Yufang in the treatment of knee osteoarthritis,and provide new ideas for clinical treatment of knee osteoarthritis.Methods:From November 2021 to October 2022,72 KOA patients who met the inclusion criteria and were diagnosed as qi stagnation and blood stasis syndrome were selected from the outpatient department of the Rheumatism and Bone Disease Center of Gansu Provincial Hospital of Traditional Chinese Medicine.Personal basic information and safety tests were completed and relevant scales were filled out.According to the random number table method,72 patients were divided into control group(36 cases)and observation group(36 cases).The control group was treated with basic treatment combined with western medicine(etoricoxib + glucosamine hydrochloride),while the observation group was treated with modified Yufang on the basis of the control group.Both groups were treated for 4 weeks.The VAS score,WOMAC total score and sub-score,Lysholm score,TCM syndrome score and pain recurrence after 1 month of withdrawal were statistically analyzed before and after treatment.The total dose of etoricoxib was recorded to evaluate the clinical efficacy and safety of Wuwei Han Dynasty Yijian Yufang in the treatment of knee osteoarthritis with qi stagnation and blood stasis.Results: A total of 72 patients with knee osteoarthritis of qi stagnation and blood stasis type were included in this study.During the study,2 cases in the control group were lost to follow-up,and 3 cases were excluded due to poor efficacy and other treatment options during the treatment.In the observation group,3 cases were lost to follow-up,and 1 case was excluded due to the inability to continue treatment.Finally,63 patients were involved in the study,including 32 in the observation group and 31 in the control group.1.Comparison of curative effect:after 4 weeks of treatment,(1)Comparison of curative effect of TCM syndromes:the total effective rate of TCM syndromes in the observation group and the control group was 84.4% and 64.5% respectively,and the difference between the two groups was statistically significant(P<0.05).(2)The total effective rates of the observation group and the control group were 75.0% and 51.6%,respectively.The difference between the two groups was statistically significant(P < 0.05).The clinical efficacy of the observation group was better.2.Comparison of scores:(1)Comparison between the two groups after 4 weeks of treatment:VAS score,WOMAC total score,pain,stiffness,daily activity score,and TCM syndrome score all decreased compared with the control group,and Lysholm score increased compared with the control group,with statistical difference(P <0.05).(2)Comparison between the two groups before and after treatment:VAS score,WOMAC total score,pain,stiffness,daily activity score,Lysholm score and TCM syndrome score were compared between the two groups before and after 4 weeks of treatment.The differences were statistically significant(P < 0.01),indicating that both treatment schemes were effective,and the observation group was more significant than the control group.3.Recurrence of pain after 1 month of withdrawal:there were 4 cases and 11 cases of recurrence in the observation group and the control group respectively,and the recurrence rates were 12.5% and 35.5% respectively.The recurrence rate of the observation group was lower,with statistical difference(P < 0.05).4.Comparison of total dose of etoricoxib: the dose of etoricoxib in the observation group was lower than that in the control group,with statistical difference(P < 0.05).5.Evaluation of adverse reactions:During the treatment of the two groups of patients,the incidence of adverse reactions in the observation group and the control group was 3.1% and9.7% respectively.Conclusion:The curative effect of modified Yufang on knee osteoarthritis of qi stagnation and blood stasis type is clear,especially in relieving pain and improving knee joint function.The recurrence rate of pain is low after 1 month of drug withdrawal,and the dosage of etoricoxib can be reduced.It is safe and worthy of further clinical application.
