backgroundSuicide is a major global public health issue,with mental illness having the greatest impact on suicide rates,especially among hospitalized patients diagnosed with mood disorders or schizophrenia,where the risk of suicide increases significantly;Among them,bipolar disorder has the highest risk of suicide among psychiatric disorders.However,there is still a lack of effective methods to reduce the incidence of suicide in hospitalized patients with mental disorders,therefore,in-depth research on the suicide risk of hospitalized patients with mental disorders is still particularly important.Our aim is to investigate the suicide risk and factors associated with high suicide risk in hospitalized patients with bipolar disorder,in order to identify the population of high-risk inpatients and implement targeted interventions early on to reduce the incidence of suicide.MethodsRetrospective analysis was conducted on patients with bipolar affective disorder discharged from A large psychiatric hospital in Guangzhou from December 1,2018 to November 30,2020.demography and clinical characteristics data were extracted,and the suicide risk of patients was assessed using the Nurses,Global Assessment of Suicide Risk Scale(NGASR).ResultsThe group with severe depressive episodes of bipolar disorder had the highest risk of suicide(p<0.01),regardless of whether they were accompanied by psychotic symptoms.The suicide risk in the mixed state group is only second to that in the severely depressed group.The suicide risk of women in the severe depressive episode group was 1.854 times higher than that of men(P=0.005),and the suicide risk of involuntary admission patients was 1.701 times higher than that of voluntary admission patients(P=0.016).The suicide risk of women in the mixed state group was 2.075 times higher than that of men(P=0.011),and the suicide risk of patients younger than 18 years old was 2.737 times higher than those between 18 and 65 years old(P=0.004).There was no statistically significant difference in suicide risk between the severely depressed group with psychotic symptoms and the severely depressed group without psychotic symptoms(p=0.270).ConclusionPatients with bipolar disorder currently in the form of severe depressive episodes and mixed clinical episodes have a significantly higher risk of suicide than those with mild or moderate depressive episodes and manic episodes.The severity of depressive symptoms is correlated with the risk of suicide;Compared to depressive symptoms,no correlation was found between psychiatric symptoms and suicide risk.Female,involuntary admission is a risk factor for increased suicide risk in patients with bipolar severe depressive episodes.Female and younger than 18 years old are risk factors for increased suicide risk in patients with bipolar mixed state.
