| Objective:By analyzing and discussing the results of HPVDNA detection,TCT detection,HR-HPVE6/E7 mRNA detection,and pathological findings in various levels of cervical lesions,the screening effect of HR-HPVE6/E7 mRNA detection on cervical precancerous lesions and cervical cancer is analyzed and discussed,providing effective reference basis for the prevention,screening,and early diagnosis of cervical precancerous lesions and cervical cancer.Methods:Collect data on patients who underwent cervical tissue biopsy under colposcopy from May 2022 to October 2022 at the Obstetrics and Gynecology Clinic of the Second Affiliated Hospital of Guangzhou Medical University who met the screening criteria for cervical cancer,including HPVDNA,TCT,and pathological results,to understand the HPV infection rate,the relationship between HPVDNA,TCT,HR-HPVE6/E7 mRNA detection and cervical precancerous lesions and cervical cancer,and analyze the results.Results:Among the 203 patients included in this study,189(93.10%)were high-risk and15(6.90%)were low-risk in HPV DNA testing.TCT detected 84 cases of NILM(41.38%),78 cases of ASCUS(38.42%),35 cases of LSIL(17.24%),and 6 cases of HSIL(2.96%).85 cases(41.87%)of HR-HPV E6/E7 mRNA were negative and 118cases(58.13%)were positive.Pathological diagnosis of chronic inflammation of the mucosa in 84 cases(41.38%),CIN Ⅰ in 56 cases(27.59%),CIN Ⅱ in 34 cases(16.75%),and CIN Ⅲ in 23 cases(11.33%);There were 6 cases(2.96%)of cervical cancer.The sensitivity,positive predictive value,and negative predictive value of HPV DNA detection are 94.12%,59.26%,and 50%,respectively.The sensitivity,specificity,positive predictive value,and negative predictive value of TCT detection are 69.75%,57.14%,69.75%,and 57.14%,respectively.The sensitivity,specificity,positive predictive value,and negative predictive value of HR-HPV E6/E7 mRNA detection were 68.91%,57.14%,69.49%,and 56.47%,respectively.The positive expression rates of HPV E6/E7 mRNA in patients with mucosal chronic inflammation,CIN Ⅰ,CIN Ⅱ,CIN Ⅲ,and cervical cancer were 42.86%,67.86%,70.59%,60.87%,and 100%,respectively.The positive rates of HR-HPV E6/E7 mRNA in high-risk HPV DNA positive patients and TCT positive patients were not statistically significant(χ~2= 0.369,P>0.05)。The three detection results of HR-HPV DNA,TCT,and HR-HPV E6/E7 mRNA were statistically significant compared with the diagnostic positive rates of ≤CIN Ⅰ and ≥ CIN Ⅱ in pathological results(P values<0.05)Conclusions:(1)The differences in HR-HPV E6/E7 mRNA,HR-HPV DNA,and TCT detection were statistically significant in cervical biopsy pathological results of ≤ CIN Ⅰ and ≥CIN Ⅱ.Among them,HR-HPV DNA detection had the highest sensitivity,and the specificity of HR-HPV E6/E7 mRNA detection and TCT detection was higher than that of HR-HPV DNA detection.(2)As the level of cervical lesions increases,the positive expression rate of HRHPVE6/E7 mRNA detection increases,but it is not sufficient for its shunt effect and cannot be used to determine the severity of cervical lesions.(3)In this study,the order of detection efficacy was TCT detection>HR HPV E6/E7 mRNA detection>HR HPV DNA detection. |
