Purpose : The occurrence and development of Acute cerebral infarction(ACI)can not be achieved without the involvement of inflammation.By studying the predictive value of red blood cell distribution width(RDW)on the severity and prognosis of ACI patients,this paper provides a new indicator that can reflect the inflammatory state,which can be used for the disease assessment and early diagnosis and treatment of ACI patients.So as to achieve primary and secondary prevention effect.Methods: 135 ACI patients admitted to XX hospital from January2022 to December 2022 were analyzed.The clinical data were retrospectively analyzed.Including general data,laboratory indicators,imaging indicators,National Institutes of Health Stroke Scale,(NIHSS),modified Rankin Scale(mRS),etc.,All the patients were diagnosed with cerebral infarction by computed tomography(CT)and/or magnetic resonance imaging(MRI).The study was divided into large core infarction volume group(> 50ml)and small core infarction volume group(≤ 50ml)according to the core infarction volume.According to NIHSS score,they were divided into high group(> 15 points)and low group(≤ 15points).According to MRS Scores,they were divided into poor prognosis group(> 2points)and good prognosis group(≤ 2points).Univariate and binary Logistic regression analyses were performed,and ROC curves were plotted to describe the predictive value of RDW for the severity and prognosis of ACI.Results:1.According to the group of core infarction volume,comparative analysis of clinical indicators of patients in the two groups showed that RDW and white bloodcell count(WBC)were higher in the large core infarction volume group,and there were statistically significant differences between groups(all P < 0.05).Logistic regression analysis showed that RDW and WBC were independent risk factors for large core infarction volume(P < 0.05).2.According to the NIHSS score,the comparative analysis of clinical indicators of the two groups showed that: The proportion of patients with heart disease,RDW and WBC were higher and the high density lipoprotein cholesterol(HDL-C)was lower in the high NIHSS group,and the differences between groups were statistically significant(all P < 0.05).Logistic regression analysis showed that heart disease and RDW were independent risk factors for high NIHSS score(P < 0.05),and HDL-C was independent protective factor for high NIHSS score(P < 0.05).3.According to mRS Scores,comparative analysis of clinical indicators of patients in the two groups showed that the proportion of hypertension and heart disease,age,RDW and WBC of patients in the poor prognosis group were higher,and HDL-C was lower,with statistical significance between groups(P < 0.05).Logistic regression analysis showed that hypertension and RDW were independent risk factors for poor prognosis(P < 0.05),and HDL-C was independent protective factor for poor prognosis(P < 0.05).4.ROC curve analysis showed that RDW predicted the area under the curve(AUC)of large core infarction volume was 0.769(P < 0.05).RDW predicted the AUC of high NIHSS score was 0.726(P < 0.05).RDW predicted that the AUC of high mRS Score was 0.665(P < 0.05).Conclusion:1.RDW level at admission of ACI patients is clearly correlated with core infarction volume and NIHSS score at admission.The higher RDW level is,the larger core infarction volume,the higher NIHSS score at admission,and the higher severity of the disease.2.RDW level at admission of ACI patients was clearly correlated with mRS Score at discharge.The higher the RDW level was,the higher the mRS Score at discharge,and the higher the risk of poor prognosis3.RDW had a certain predictive value for large core infarction volume and high NIHSS score in ACI patients,but had a low predictive value for poor prognosis.
