OBJECTIVESThis study investigated the clinical effects of Chinese medicine five elements music based on constitution combined with biofeedback electrical stimulation therapy in the treatment of female stress urinary incontinence to provide a basis and reference model for improving negative emotions,relieving incontinence symptoms,improving quality of life and enhancing pelvic floor muscle function in patients with stress urinary incontinence.METHODSThis study is a prospective,randomized controlled study,the research subjects are from female stress urinary incontinence patients who visited a tertiary hospital in Guangdong from July 2022 to March 2023,and 56 patients were included according to the sample size,and 28 cases were divided into control group and 28 cases in the experimental group by random number method.The control group was treated with biofeedback electrical stimulation therapy for 30 minutes each time,with an interval of2-3 days between treatments,10 times per session;the experimental group was treated with the five elements music combined with biofeedback electrical stimulation therapy based on the somatic view.According to the patient’s constitution type,the corresponding five-tone repertoire is given,and the five-element music is listened to for 30 minutes at the same time as each biofeedback electrical stimulation treatment,and the patient can listen to it at home during the treatment interval,and the researcher regularly reminds through We Chat,10 times per course of treatment.During the treatment period,negative mood(anxiety,depression)symptoms,incontinence questionnaire,quality of life and pelvic floor muscle function were assessed before and after the intervention.Statistical analysis of the data was carried out using SPSS 27.0,and statistical significance was considered at with P<0.05.RESULTSOne patient in the control group and one in the trial group were excluded for personal reasons due to their inability to complete the 10 interventions.54 patients(96.4%)completed the full course of treatment and were included in the data analysis.The baseline comparison of general information between the two groups of patients was comparable(P> 0.05).The results of the post-intervention outcome indicators were as follows:1.Comparison of self-rating anxiety scale(SAS)scoresThe difference in SAS scores between the two groups of patients before the intervention was statistically significant(P=0.047).The difference was not statistically significant in the control group before and after comparing themselves(P=0.341).There was a statistically significant difference between the trial group’s own pre and post comparison(P<0.001).After correcting for the effect of pre-intervention SAS values using a linear regression model,the mean difference was 3.48 points lower in the trial group than in the control group,a statistically significant difference(P=0.022).2.Comparison of self-rating depression scale(SDS)scoresThe difference in SDS scores between the two groups patients before the intervention was not statistically significant(P>0.05).The difference between the control group and the trial group before and after the intervention itself was statistically significant(P<0.05).The scores of before and after the intervention in the trial group was higher than the control group,and the difference was statistically significant(P=0.010).3.Comparison of international consultation on incontinence questionnaire short form(ICI-Q-SF)scoresThe difference in ICI-Q-SF scores between the two groups of patients before the intervention was not statistically significant(P>0.05).The postintervention ICI-Q-SF scores of both groups were lower than the preintervention scores,with statistically significant differences(P<0.05).The difference in ICI-Q-SF scores between the two groups of patients before and after the intervention was not statistically significant(P=0.896).4.Comparison of incontinence quality of life questionnaire(IQOL)scoresThe difference in IQOL scores between the two groups before the intervention was not statistically significant(P>0.05).The postintervention IQOL scores were higher than the pre-intervention scores in both groups,with statistically significant differences(P<0.05).There was no statistically significant difference between the two groups in the IQOL scores before and after the intervention(P=0.283).5.Comparison of pelvic floor surface EMG parametersThe differences in class I,class II and maximal muscle potential values between the two groups pre-intervention were not statistically significant(P>0.05).After the intervention the control group had higher class I and maximal muscle potential values than b the pre-intervention,with statistically significant differences(P<0.05).Class I,class II muscle and maximal muscle potential values were higher in the test group than pre-intervention,with statistically significant differences(P<0.05).There was no statistically significant difference between the two groups in the values of class I,class II muscle and maximal muscle potential before and after the intervention(P>0.05).6.Comparison of pelvic floor modified oxford muscle strengthThere was no statistically significant difference in the Class I and Class II muscle strength levels between the two groups of patients before and after the intervention(P>0.05).After intervention,the muscle strength level of class I and class II muscle strength increased overall,and the difference between themselves before and after comparison was statistically significant(P<0.001).CONCLUSIONSThe combination of biofeedback electrical stimulation therapy with Chinese medicine five elements music based on constitution has a synergistic effect in the clinical treatment of female stress urinary incontinence patients,improving the symptoms of urinary incontinence,improving the quality of life and enhancing the function of the pelvic floor muscles while being more effective in improving negative emotions(anxiety and depression)than biofeedback electrical stimulation therapy alone.In addition,the treatment does not increase the cost of treatment for patients and is not limited by time or place,so it is worthwhile to promote the use of this treatment in clinical practice.
