Clinical Observation Of Jinhuang San In The Treatment Of Ankylosing Spondylitis Achilles Tendon Telangiectasia

ObjectiveThe purpose of this study was to observe the clinical efficacy of external application of Jinhuang powder in the treatment of ankylosing spondylitis Achilles tendinitis(damp-heat obstruction)patients in the active stage,and to compare the changes of erythrocyte settling rate,C-reactive protein,total score of TCM symptoms,ultrasonic overall improvement rate and other indicators before and after treatment in the two groups,so as to provide a new therapeutic idea for the treatment of ankylosing spondylitis Achilles tendinitis with traditional Chinese medicine.MethodsOutpatient and inpatient patients diagnosed with acute ankylosing spondylitis with Achilles tendinitis(damp-heat obstruction type)in the rheumatology Department of Foshan Hospital of Traditional Chinese Medicine from January 2022 to January 2023 were randomly enrolled.A total of 64 cases were randomly divided into two groups with 32 cases in each group.The control group was given salazopyridine enteric-coated tablets and diclofenac sodium sustained-release tablets;The treatment group was additionally treated with Jinhuang powder for external use on the basis of the control group,and each course was 2 weeks,a total of 1 course of treatment.Erythrocyte settlement rate(ESR),C-reactive protein(CRP),total score of TCM symptoms and overall improvement rate of ultrasound were compared between the two groups before and after treatment,and adverse reactions occurred during the study were recorded.The clinical efficacy and safety of external application of Jinjinsan in the treatment of acute ankylosing spondylitis with Achilles tendinitis(damp-heat obstruction type)was comprehensively evaluated.Results(1)Baseline indicators(such as age,gender,course of disease)were not statistically significant between the two groups(P>0.05).(2)There was no statistical significance in ESR and CRP of the two groups before treatment(P>0.05).After treatment,ESR and CRP indexes of the two groups were improved compared with those before treatment,and the improvement degree of the treatment group was significantly better than that of the control group,with statistical significance(P<0.05).(3)There was no statistical significance in TCM symptom scores of the two groups before treatment(P>0.05).After treatment,TCM symptom scores of the two groups were significantly lower than before treatment(P<0.05),and the TCM symptom scores of the treatment group were significantly lower than those of the control group(P<0.05).(4)Comparison of TCM efficacy between the two groups: after treatment,the total effective rate of the treatment group was 93.4%;The total effective rate of the control group was 73.3%,and the clinical effect of the treatment group was better than that of the control group,with statistical significance(P<0.05).(5)Comparison of the overall improvement rate of Achilles tendon ultrasound between the two groups: after treatment,the total effective rate of the treatment group was 86.7%;The total effective rate of control group was 60.0%.The overall improvement rate of ultrasound in the treatment group was significantly better than that in the control group,with statistical significance(P<0.05).ConclusionExternal application of Jinhuang Powder can significantly reduce the inflammatory indicators of Achilles tendinitis in active period of ankylosing spondylitis,traditional Chinese medicine syndrome score,ultrasonic overall improvement rate of Achilles tendon lesions,significantly improve the symptoms of patients,high safety,less adverse reactions,simple and effective.
