The Current Situation Of Community Rehabilitation Resources In Yunnan Province

Back ground:Due to the late start,rehabilitation therapy in China faces a twin challenge: a rapidly increasing aging population and the need for rehabilitation services from the aging population.In recent years,China’s government issued several policies to support secondary,community health care services in China to expand the rehabilitation medical team.However,most of the patients could not receive formal rehabilitative therapy.Objective:From the perspective of managers and general practitioners,our study will analyze the relationship between the development of social health work and rehabilitation resources in the process of community health development.We also clarify the wishes and influencing factors of general practitioners during the process of transformation into rehabilitation physicians.It provides theoretical support for policy formulation and decision guidance and provides practical guidance for improving the social health service environment and improving the quality of social health service in Yunnan.Methods:This study was based on our previous work,the formal investigation process is from December 2019 to March 2020.The survey uses the method including quantitative and qualitative research.Stratified cluster random sampling was used to select the community health care institutions under the supervision of Yunnan Provincial Health Commission.A total of 124 community health service centers,township health centers,and other institutions were randomly selected in our study.Six hundred and ninety-three general practitioners who were working in these institutions also participate in the questionnaire survey.All of the data were checked the exploratory analysis(EFA)and confirmatory analysis(CFA)to find the causal relationship between factors.The BM SPSS 26.0 software and AMOS24.0 software were used for data analysis.Results:Among the 123 complete questionnaires,ninety(75%)of medical institutions set up rehabilitation departments or traditional Chinese medicine rehabilitation departments.As many as 93.33% of managers show that there was a shortage of rehabilitation professionals,especially rehabilitation specialists.The Cronbach’s alpha value of the managers’ questionnaire is 0.783,which means good internal consistency.The seven factors extracted can explain 69.777% of the total variation.The structural model of the four factors retained by the SR model after MI adjustment and CFA has a high degree of fit.The interpretation of the results of the structural model suggests as follows.The shortage or not of rehabilitation personnel is directly shown when trying to carry out rehabilitation projects.The options of carrying out some difficult or common diseases and the options of shortage of rehabilitation personnel are reciprocally influenced by each other.A total of 670 general practitioner questionnaires were included in this study.The Cronbach’s alpha value of the general practitioner questionnaire is 0.801,which has a good internal consistency.The six factors extracted from the general practitioner questionnaire can explain the total variation rate of 61.129%.The CFA of the SR model adjusted by MI indicates that the structural model of all factors has a high degree of fit.The interpretation of the results of the general practitioner questionnaires model suggests as follows.The option that whether the general practitioners are willing to undertake the role of rehabilitation specialists is directly affected by the option that patients showed rehabilitation needs on difficult diseases and indirectly affected by needs on common diseases.That willing also indirectly affected by the pathways to increase professional skills.Moreover,whether the general practitioner will recommend rehabilitation therapy actively is influenced by the rehabilitation needs of patients on common diseases.Conclusion(s):The lack of community rehabilitation resources in Yunnan province is mainly reflected in the lack of rehabilitation professionals,especially rehabilitation specialists,which affects the development of rehabilitation projects and diseases.As general practitioners who are mainly responsible for rehabilitation doctors in the future,most of them are willing to assume the role of community rehabilitation specialists,but they need to increase more opportunities to upgrade their professional skills in order to alleviate the professional ability problems.Moreover,the active demand by patients for rehabilitation needs can promote further general practitioners to undertake the role of rehabilitation physicians.
