| Objective:To describe the status of fall efficacy in hospitalized patients in geriatrics department and define the characteristics of fall efficacy.To analyze the influencing factors of fall efficacy of elderly inpatients in order to reduce the incidence of falls and provide objective basis for health care workers to make intervention measures to prevent falls.Methods:By means of cross-sectional investigation and phenomenological research,300 in-patients in geriatrics department of four Grade III hospitals in Beijing were conveniently selected from January 2019 to June 2020 to study the present situation of fall efficacy and its influencing factors.This study is based on self-efficacy theory,the instruments included the general and disease questionnaire,the Hospital environmental impact factors questionnaire,the modified fall effectiveness Scale(MFES),the BARTHEL index Scale(BI),the fall risk assessment Scale(MFS),the Social Support Rating Scale(SSRS),the Frailty Phenotype(FP),and the Hospital Anxiety and depression Scale(HADS).In this study,17 hospitalized patients in geriatrics department were selected for semi-structured interviews to analyze the internal causes and related factors that affect the patients’fall efficacy,to explore the deeper perception and experience of patients’fall efficacy in quantitative research,and to explore the deeper basis of influencing patients’fall efficacy judgment,and to supplement and explain the quantitative results.Use Excel to organize the data,use SPSS 22.0 statistical software to analyze the data,methods:descriptive statistical analysis,Colaizzi method,independent sample t test,analysis of variance,single factor correlation analysis,multiple Regression analysis,structural equation analysis,etc.Result:1.The average score of fall efficacy was(6.970 ± 2.980),and the highest score of 14 activities on the fall efficacy scale was "changing clothes"(8.783 ± 2.090),the lowest score was for "use of public transport"(5.573±4.105).2.The results showed that there were significant differences in 15 indexes including age,BMI,mode of medical payment,education level,exercise,fall experience,failure cases,types of medication,general health status,chronic diseases,depression,social support,weakness,daily living ability and fall risk among different groups(p<0.05).3.Multiple Regression analysis were available,fall risk,ADL,weakness,depression,and failure cases were the important influencing factors of fall efficacy(β>0,p<0.05).The Complex Correlation Coefficient R=0.828 and the determinative Coefficient R2=0.686 of the regression equation model.4.The results of semi-structured interview showed that 6 subjects,including physical health,lower limb and muscle balance,exercise,fear of adverse consequences after falls,the influence of falls on others,and caretakers’fear of responsibility,were the deeper basis for judging the fall efficacy of elderly inpatients.Conclusion:1.The fall efficiency of senile inpatients was medium level.2.The main influencing factors of fall efficacy of geriatric inpatients were:Ability of daily living,risk of fall,weakness,depression,and self-similar failure to prevent fall.3.In the future,we should pay more attention to the evaluation of the fall efficacy of the elderly patients,comprehensively evaluate the factors affecting the fall efficacy,and give individualized intervention measures to reduce the occurrence of falls. |
