Objective:This study is a single-blind,randomized controlled study to evaluate the efficacy and safety of ’Huoxue Sanfeng ’ acupuncture on the clinical symptoms and vascular function of subjects with prehypertension(PHT)from different perspectives of Chinese and Western medicine.It provides new ideas and methods for the prevention and treatment of PHT with reliable curative effect and safe operation.Methods:In this study,64 subjects with PHT(BP:120~139mmHg and/or 80~89mmHg)were recruited from the outpatient department of the The Third Affiliated Hospital of Beijing University of Chinese Medicine and the surrounding communities.They were randomly divided into observation group and control group with 32 people in each group.In the control group,28 cases(4 cases lost)were actually observed,and health education was given to guide the improvement of lifestyle.The observation group actually observed 29 cases(3 cases fell off),and intervened with ’ Huoxue Sanfeng ’ acupuncture method on the basis of health education.They were treated at least 3 times a week for 4 weeks,a total of 12 times.Before and after the intervention,the blood pressure of the two groups was measured,and the TCM syndrome scale of hypertension,the Prediction for ASCVD Risk in China(China-PAR)and the ultrasonic qq technique were used to measure the related indexes of vascular function.Results:1.Inclusion and shedding:A total of 64 PHT subjects were included in this study.For various reasons,the final effective observation object was 57 cases,including 28 cases in the observation group and 29 cases in the control group.2.Comparison of general data:There was no significant difference in age,gender,personal history(family history of hypertension,smoking history,drinking history),abdominal circumference and BMI between PHT subjects who met the inclusion and exclusion criteria(P>0.05).3 Efficacy indicators3.1 Blood pressure in the office:①Blood pressure value:There was no difference in baseline blood pressure between the two groups(P>0.05).Compared with before treatment,systolic blood pressure and diastolic blood pressure decreased in both groups(P<0.01).There was a significant difference in systolic blood pressure between the two groups after treatment(P<0.01),and there was no significant difference in diastolic blood pressure(P>0.05).② Antihypertensive effective rate:The total effective rate of the observation group was 64.29%.The total effective rate of the control group was 31.03%.Compared with the control group,the total effective rate of blood pressure reduction in the observation group was significantly higher,that is,the total effective rate of the two groups after treatment was significantly different(P<0.01).3.2 TCM Syndrome Scale:①TCM syndrome scale score:Compared with before intervention,the TCM syndrome scores of the two groups were decreased after intervention,and the difference was significant(P<0.05).②Effective rate of TCM syndrome integral improvement:The total effective rate of the observation group was 82.14%.The total effective rate of the control group was 58.62%.Compared with the control group,the total effective rate of the observation group was significantly higher.There was a significant difference in the total effective rate between the two groups after intervention(P<0.01).③Specific symptoms of TCM syndrome scale:There was no significant difference in the symptoms of the two groups of subjects in the baseline TCM syndrome scores except headache,knee softness,vomiting,sputum and saliva(P>0.05).After intervention,the symptoms of vertigo,head weight,chest tightness,pale mouth,less food,red face,dry mouth,palpitation and insomnia were improved in both groups(P<0.05).The head weight,less food and palpitation in the observation group were significantly different from those before treatment(P<0.01).In addition,the scores of irritability and constipation in the observation group were also significantly different after intervention(P<0.05,P<0.01).After intervention,the improvement of head weight and dry mouth in the observation group was significantly different from that in the control group(P<0.05).3.3 Carotid vascular functionThe bilateral carotid intima-media thickness and vascular elasticity related indexes of the observation group after intervention were lower than those before treatment,and the difference was significant(P<0.01).In the control group,the intima-media thickness of bilateral vessels,the right carotid artery cc and the left carotid artery α were lower than those before treatment,and the differences were statistically significant(P<0.05,P<0.01).2.Comparison between the two groups after intervention,the improvement of right carotid artery intima-media thickness,bilateral expansion coefficient,left compliance coefficient and pulse wave velocity in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05,P<0.01).3.4 ASCVD risk prediction model(China-PAR)The ten-year risk and lifetime risk of the two groups after intervention were lower than those before treatment,and the difference was significant(P<0.01).Compared between the two groups,the ten-year risk of the observation group after intervention was lower than that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the lifelong risk between the two groups after intervention(P>0.05).Conclusion:1.The ’Huoxue Sanfeng’ acupuncture method has a significant antihypertensive effect on high-normal blood pressure,and can safely and effectively improve clinical symptoms and reduce the risk of ASCVD for 10 years.2.The ’ Huoxue Sanfeng ’ acupuncture method can effectively reduce the carotid artery intima-media thickness and improve vascular elasticity.
