Objective:Comparative study on the sensitivity,specificity,and accuracy of CT plain scan indicators,namely,Peritoneal to peritoneal height ratio(PAR),Balthazar CT grade,and Extrapancreatic Inflammation on Computed Tomography(EPIC),in early assessment of the severity of acute pancreatitis,organ infection,admission to ICU,and death.At the same time,explore the value of combining the new indicator PAR with leukocytes,hypersensitive C-reactive protein,calcium ions,and urea nitrogen in evaluating the severity of acute pancreatitis.Methods:The clinical,laboratory,and imaging data of 221 patients with acute pancreatitis hospitalized in Baise People’s Hospital from August 2021 to August 2022 were collected.Retrospective analysis and comparison of PAR,Balthazar CT grading,EPIC and other CT scan indicators were performed to assess the severity of acute pancreatitis.Use the area under the curve(AUC)of ROC to evaluate the value of various CT indicators in assessing the severity of acute pancreatitis,organ infection,admission to the ICU,and death within the same time period.Compare the area under the curve(AUC)of each indicator to determine its sensitivity,specificity,and accuracy.At the same time,explore the value of new indicators PAR combined with leukocytes,hypersensitive C-reactive protein,calcium ions,and urea nitrogen in evaluating the condition of acute pancreatitis.Results:In the first CT scan on admission,EPIC had the highest accuracy in assessing acute pancreatitis severity stratification,organ infection,admission to ICU,and death among CT scan indicators(AUC=0.744,AUC=0.807,AUC=0.850,AUC=0.820).The new indicator PAR combined with biochemical indicators such as leukocyte,CRP,calcium ion,and urea nitrogen has a higher accuracy in assessing severe stratification and organ infection in acute pancreatitis(AUC=0.750,AUC=0.752);The accuracy of PAR combined with urea nitrogen was higher when assessing acute pancreatitis admission to ICU and death(AUC=0.791,AUC=0.978);The accuracy of combining PAR with CRP and urea nitrogen in assessing severe stratification of acute pancreatitis,organ infection,admission to ICU,and death was improved(AUC=0.776,AUC=0.781,AUC=0.809,AUC=0.981).Conclusion:In early CT scans,EPIC evaluation has a high accuracy in assessing acute pancreatitis severity stratification,organ infection,admission to the ICU,and death.The new CT index PAR combined with CRP and urea nitrogen has high accuracy in evaluating the condition of acute pancreatitis,and can be used as an early and rapid indicator for evaluating the condition of acute pancreatitis.
