Exploring The Distribution Of TCM Syndromes And Medication Rules For Chronic Heart Failure With Preserved Ejection Fraction (HFpEF) Based On Data Mining Technolog

Objective : To explore the distribution law of Traditional Chinese Medicine(TCM)syndrome type and medication rule of chronic heart failure with ejection fraction retention(HFp EF)by data mining technology,and to provide reference for clinical TCM treatment.Method : Collect all relevant literatures from CNKI,Wanfang Data Knowledge Service Platform,VIP Chinese periodicals,and Pub Med from 1995 to January 2023.Collate literatures that meet the standards according to the inclusion criteria and the exclusion criteria.Input the data to the“statistical management”module of TCM inheritance assistance platform successively,and the“statistical report”and “data analysis”modules were used to analyze to the data.Results : A total of 110 literatures met the criteria,and 110 non-repetitive TCM prescriptions were obtained.Through data analysis,we found that:(1)Analysis of TCM syndrome types : A total of 32 types of syndromes are involved.The syndrome of Qi deficiency and blood stasis was the most distributed,followed by the syndrome of Yang deficiency and Blood stasis and Water-dampness,the syndrome of Qi and Yin deficiency and Blood stasis,the syndrome of Yang deficiency and Water-dampness,and the syndrome of Qi and Yin deficiency.Qi deficiency and Blood stasis mixed with Water-dampness,Yin deficiency,Yang deficiency,the total of four syndrome accounted for more than 46%.(2)Analysis of TCM syndrome elements : The location of the disease is mainly in the heart,involving the lung,kidney and spleen.There are 8 basic syndromes in total.The distribution of blood stasis was the most,followed by Qi deficiency.80% of the pathogenic cases were caused by two or three syndromes,and nearly half of the cases were double syndromes.The least number of cases were single syndrome elements.(3)Overall medication law : A total of 156 the Chinese medicinal herbs were involved.The prescriptions with the lowest number of the Chinese medicinal herbs were only 2,while the prescriptions with the highest number consisted of 18 Chinese medicinal herbs.The Chinese medicine were divided into 11 categories according to their efficacy,with the largest number of nourishing Qi,followed by drugs for invigorating blood circulation and eliminating stasis,preventing phlegm and relieving cough and asthma.The Chinese medicine from high to low frequency,there are Salvia miltiorrhiza,Astragalus membranaceus,Poria cocos,Ligusticum wallichii,Cassia twing,Semen lepidii,etc.The medicinal properties are mainly warm,cold and flat.The main medicinal tastes are sweet,bitter and tangential.The main channels are heart,spleen,lung and liver.(4)There were 67 prescriptions for treating Qi deficiency syndrome,involving 119 Chinese medicine.From high to low,They were Salvia miltiorrhiza,Astragalus membranaceus,Ligusticum wallichii,Ginseng and so on.There were 18 drug combinations with frequency≥13,and the core drugs included 9 Chinese medicine.A total of 22 drugs rules with a confidence level above 0.6.(5)There were 39 prescriptions for treating Yang deficiency syndrome,involving 84 Chinese medicine.From high to low,They were Salvia miltiorrhiza,Poria cocos,Astragalus membranaceus,Radix aconiti carmichaeli and so on.There were 59 drug combinations with frequency≥8,and the core drugs included 10 Chinese medicine.A total of 236 drugs rules with a confidence level above 0.6.(6)There were 23 prescriptions for treating Yin deficiency syndrome,involving 75 Chinese medicine.From high to low,They were Radix Ophiopogonis,Salvia miltiorrhiza,Schisandra chinensis,Astragalus membranaceus and so on.There were 99 drug combinations with frequency≥5,and the core drugs included 12 Chinese medicine.A total of 83 drugs rules with a confidence level above 0.6.(7)There were 85 prescriptions for treating Blood stasis syndrome,involving 133 Chinese medicine.From high to low,They were Salvia miltiorrhiza,Astragalus membranaceus,Ligusticum wallichii and so on.There were 23 drug combinations with frequency≥16,and the core drugs included 12 Chinese medicine.A total of 28 drugs rules with a confidence level above0.6.(8)There were 34 prescriptions for treating water-dampness syndrome,involving 81 Chinese medicine.From high to low,They were Poria cocos,Astragalus membranaceus,Salvia miltiorrhiza,Semen lepidii,Rhizoma atractylodis macrocephalae and so on.There were 36 drug combinations with frequency≥7,and the core drugs included 10 Chinese medicine.A total of 45 drugs rules with a confidence level above 0.6.(9)There were 18 prescriptions for treating Phlegm-turbid syndrome,involving 83 Chinese medicine.From high to low,They were Salvia miltiorrhiza,Astragalus membranaceus,Panax notoginseng,Trichosanthes kirilowii Maxim and so on.There were 17 drug combinations with frequency≥4,and the core drugs included 10 Chinese medicine.A total of 18 drugs rules with a confidence level above 0.6.(10)The core drugs for the treatment of Qi deficiency and Blood stasis syndrome include Astragalus membranaceus,Salvia miltiorrhiza,Ginseng,Angelica sinensis,Ligusticum wallichii,Poria cocos,Cassia twing,Rhizoma atractylodis macrocephalae and Honey-fried licorice root.The core drugs for the treatment of Yang deficiency and Water-dampness syndrome include Astragalus membranaceus,Salvia miltiorrhiza,Radix aconiti carmichaeli,Cassia twing,Poria cocos,Semen lepidii,Rhizoma atractylodis macrocephalae.The core drugs for the treatment of Yang deficiency and Blood stasis and Water-dampness syndrome include Salvia miltiorrhiza,Astragalus membranaceus,Poria cocos,Semen lepidii,Rhizoma atractylodis macrocephalae,Rhizoma alismatis,Cassia twing,Radix aconiti carmichaeli,Safflower carthamus,Radix ginseng Rubra.The core drugs for the treatment of Qi and Yin deficiency syndrome include Radix Ophiopogonis,Honey-fried licorice root,Salvia miltiorrhiza,Schisandra chinensis,Astragalus membranaceus,Radix rehmanniae recen,Cassia twing,Fructus Cannabis,Ginseng,Poria cocos,Rhizoma alismatis.The core drugs for the treatment of Qi and Yin deficiency and Blood stasis syndrome Salvia miltiorrhiza,Radix Ophiopogonis,Astragalus membranaceus,Radix paeoniae rubra,Radix pseudostellariae,Radix rehmanniae recen,Lilium brownii,Ligusticum wallichii,Leonurus,Angelica sinensis,Lumbricus.(11)A total of 10 potential drug combinations were obtained by cluster analysis.Conclusion : HFp EF is always the syndrome of deficiency of origin and excess of substance.The deficiency is Qi deficiency,Yang deficiency,Yin deficiency and Blood deficiency.And the sthenia-syndrome includes Blood stasis,Water-dampness,Phlegm-turbid and Qi stagnation.According to different combinations of syndrome elements in treatment,giving symptomatic support treatment such as invigorating Qi,tonifying Yin and Yang,promoting blood circulation and removing blood stasis,dispelling dampness and eliminating phlegm,guiding Qi stagnation,etc.
