Objective:To observe the clinical efficacy of warm needling at Tianshu(ST 25)in the treatment of Diarrhea-predominant Irritable bowel syndrome(IBS-D),and reveal the relationship between different moxibustion doses and curative effect,so as to provide a basis for optimizing the treatment plan of IBS-D with warm needling treatment.Methods:A total of 60 IBS-D patients with liver stagnation and spleen deficiency syndrome who met the inclusion criteria were numbered according to the time sequence of treatment.According to the random number table method,the patients were divided into trial group 1(warm needling with one-cone moxibustion to ST25),trial group 2(warm needling with two-cone moxibustion to ST25)and control group(simple acupuncture)according to the ratio of1:1:1,20 cases in each group.A total of 55 effective cases were finally completed,including 18cases in trial group 1,19 cases in trial group 2 and 18 cases in control group.The treatment was given once every other day,3 times a week for 4 consecutive weeks,a total of 12 times.The TCM clinical symptom score,IBS-QOL and HAMA were recorded before and after treatment in the three groups,and the therapeutic effects were compared between the groups.Results:1.Comparison of TCM clinical symptom scores among the three groups:(1)Compared with before treatment,TCM clinical symptom scores of the three groups decreased after treatment,and the differences were statistically significant(P<0.01),indicating that different moxibustion doses of warm needling and simple acupuncture can significantly improve the clinical symptoms of IBS-D patients.(2)The difference between the three groups(pre-treatment to post-treatment)was compared with the control group,and the difference between trial group 1 and trial group 2was statistically significant(P<0.01);Compared with trial group 1,the score of trial group 2decreased more significantly,and the difference was statistically significant(P<0.01);The results showed that the effect of warm needling at Tianshu point doubled the amount of moxibustion(two-cone moxibustion)>the amount of conventional moxibustion(one-cone moxibustion)>single acupuncture without moxibustion.2.Comparison of IBS-QOL scores among the three groups:(1)Compared with before treatment,IBS-QOL scores of all dimensions and total scores of the three groups were increased,and the differences were statistically significant(P<0.01),indicating that different moxibustion doses of warm needling and simple acupuncture can improve the quality of life of patients with IBS-D.(2)Comparison of the difference between the three groups(post-treatment and pre-treatment):in Q1,Q4 and Q5 dimensions and total scores,trial group 1 and trial group 2 were compared with the control group,and the differences were statistically significant(P<0.01);Compared with trial group 1,the score of trial group 2 increased more significantly in Q1,Q4 and Q5 dimensions and total score,and the differences were statistically significant(P<0.05).The results showed that in terms of improving patients’irritability,diet and health concerns,the effect of warm needling at Tianshu point doubled the amount of moxibustion(two-cone moxibustion)>the amount of conventional moxibustion(one-cone moxibustion)>single acupuncture without moxibustion.In Q2 dimension,there was no significant difference between trial group 1 and control group(P>0.05),the difference between trial group 2 and control group was statistically significant(P<0.05);Compared with trial group 1,Q2 scores of trial group 2 increased more significantly,and the difference was statistically significant(P<0.05);The results showed that the effect of warm needling at Tianshu point was double moxibustion amount(two-cone moxibustion)>conventional moxibustion amount(one-cone moxibustion),and the effect of conventional moxibustion amount(one-cone moxibustion)was comparable to that of single acupuncture without moxibustion.In Q3,Q6,Q7 and Q8 dimensions,the difference between the three groups was pairwise compared,and the difference was not statistically significant(P>0.05),indicating that the efficacy of warm needling with different amount of moxibustion was comparable to that of single acupuncture in improving patients’body thought,social response,concept of opposites and interpersonal relationship.3.Comparison of HAMA scores among the three groups:(1)Compared with before treatment,HAMA scores of the three groups after treatment decreased,and the differences were statistically significant(P<0.01),indicating that different moxibustion doses of warm needling and simple acupuncture can significantly improve the anxiety of patients with IBS-D.(2)The difference between the three groups(pre-treatment to post-treatment)was compared,and the difference between trial group 1 and control group was statistically significant(P<0.05),the difference between trial group 2 and control group was statistically significant(P<0.01);Compared with trial group 1,the score of experiment group 2 decreased more significantly,and the difference was statistically significant(P<0.01);The results showed that the effect of warm needling at Tianshu point doubled the amount of moxibustion(two-cone moxibustion)>the amount of conventional moxibustion(one-cone moxibustion)>single acupuncture without moxibustion.4.Comparison of the efficacy of three groups of TCM clinical symptoms:the total effective rate of trial group 1 is 88.89%,the total effective rate of trial group 2 is 94.74%,the total effective rate of control group 72.22%,the use ofχ~2test for statistics,trial group 1,trial group 2 compared with the control group,the difference is statistically significant(P<0.01);The difference between trial group 2 and trial group 1 was statistically significant(P<0.05),indicating that the curative effect of double moxibustion amount(two-cone moxibustion)>conventional moxibustion amount(one-cone moxibustion)>single moxibustion without moxibustion in the treatment of IBS-D,preliminary evidence that there is a certain correlation between the amount of moxibustion and clinical efficacy.Conclusion:Warm needling at Tianshu Point can significantly improve gastrointestinal symptoms such as diarrhea and abdominal pain in patients with IBS-D,relieve negative emotions such as irritability and anxiety,improve diet status and daily life quality.The clinical effect of warm needling at Tianshu Point(two-cone moxibustion)is better than that of conventional moxibustion at Tianshu point(one-cone moxibustion)and better than that of single acupuncture without moxibustion,and the effect of double moxibustion at Tianshu point(two-cone moxibustion)is better.The preliminary evidence shows that there is a certain correlation between the amount of moxibustion and clinical effect,which provides a certain basis for clinical use of warm needling at Tianshu Point(two-cone moxibustion)to treat patients with IBS-D.This therapy is safe and worthy of clinical promotion.
