Below are main steps of present study:part one: fundamental part: ①model of middle cerebral artery occlusion(MCAO) was made on Wistar rats by modified thread occlusion;②The rats were divided into 3 groups: acupuncture treatment group(group A);infarction contral group(group B) and normal contral group(group C); Acupuncture was practiced as soon as the rats awenkened,electric acupuncture were applied at bilateral Neiguan and Zusanli points, choosing a intensity according to the rat's limb vibration. Even needling method was applied at Baihui points, the suitable intensity was achieved with a sinking feeling.The 1-day-after- ischemia goup accepted acupuncture once per 24 hours respectively. Observation were made about acupuncture's influence on MCAO rat's behavior recovery;③The expression of MAP2 in the cortex ipsilateral and contralateral to the lesion was determined among group A and B at 1d,3d,7d and 14d after the lesion with immunohistochemistry. The result was compared with that of normal rats and acupuncture's effect on dendrite plasticity was explored. ④The expression of SYN in the cortex ipsilateral and contralateral to the lesion was determined among group A and B at 1d,3d,7d and 14d after the lesion with immunohistochemistry.The result was compared with that of normal rats and acupuncture's effect on dendrite plasticity was explored. ⑤The expression of GAP43 mRNA in the cortex ipsilateral and contralateral to the lesion was determined among group A and B at 1d,3d,7d and 14d after the lesion with in situ hybridization.The result was compared with that of normal rats. Acupuncture's effect on GAP43mRNA expression and the latter's role in plasticity of neurology system were explored. ⑥The expression of nestinmRNA in the cortex ipsilateral and contralateral to the lesion was determined among group A and B at 1d,3d,7d and 14d after the lesion with in situ hybridization.The result was compared with that of normal rats. Acupuncture's effect on nestin mRNA expression and the latter's role in plasticity of neurology system were explored.Above studies laid a theory foundation for future designing of experiment about rehabilitation and acupuncture treatment after cerebral infarction. Part two:clinical trial with acupuncture 1. treatment group: (1) therapy method: the treatment method with acupuncture and facilitating technique. (2) selection of acupoint:①For the patients with increased muscular tone and abnormal motor pattern,the treatment target in early stage is to evoke motor ability on their Impinged limbs and facilitate collaborate movement. For upper limbs the acupuncture sites were chosed at flexor side such as Xuanmen,Chize,Neiguan,Lieque,and Laogong pionts .while for the lower limb, extensor sided points such as Baiguan,Futu,Zusanli,Xiexi were chosed.During later stage,the target is to facilitate separated movement,so the upper limb sites were chosed at the extensor side,namely Quchi,Shousanli,Waiguan,Hegu.,while the lower limb sites were Huantiao,Chengfu,Weizhong,Chengshan,Kunlun,Yongqian at the flexor side. ②For the patients with decreased muscular tone,the acupuncture points were chosed at both the flexor and the extensor sides in order to balance the restoration of muscle strength and tone; (3) maneuver: ①Painless insertion: inserting the needle while holding the needle in the short end ,with quick pricking.②lifting and thrusting,twirling or rotating,use twirling as the main method for provocating the needle sensation.③use strong intensities of stimulation.④no retention of the needle in the acupoint,a course of treatment last for 10 days.2. control group: (1) therapy method: select mainly the points of yang-ming meridians with supplement of points in tai-yang and shao-yan meridans (2) choosing of the acupuncture sites:①Quchi,Shousanli,Waiguan,Hegu were chosed in upper limb and Huantiao,Yanglingquan,Futu,Zusanli,Fenglong,Xiexi,Kunlun in lower limb.
