Objective:This study aimed to investigate the clinical MS-initiating factor in Dezhou subjects through multi-layered analysis of MS components, which are enlarged waist circumference, elevated blood pressure and glucose levels, increased blood triglycerides level and decreased high-density lipoprotein cholesterol concentration, and compare the importance of each component in the onset of metabolic syndrome(MS), therefore to clinical prevention for MS.Methods:1. The staffs of Dezhou authorities were recruited as the study subjects. Using the diagnosis standard of international diabetes federation (IDF,2006) to diagnose MS components-enlarged waist circumference (male≥90cm, female≥80cm), elevated blood pressure (systolic pressure≥130 mm Hg or diastolic≥85mmHg), high levels of blood glucose (fasting glucose≥5.6 mmol/L) and high triglycerides (≥1.7 mmol/L), decreased high-density lipoprotein cholesterol concentrations (HDL-c) (male<1.03mmol/L; female<1.29 mmol/L).2. The investigation showed that MS incidence is 41.3% in male and 8.4% in female and there were few female with MS. The later study mainly focused on Han male subjects. All male individuals were divided into obese group (waist circumference≥90cm, n=370) or the non-obese group (waist circumference<90cm, n=228), The incidences of MS and its risk factors were analyzed and compared between different age groups. 3. A total of 136 male patients with MS were randomly divided into only on diet control group (group A, n=45), only exercise group (group B, n=45) or diet-control plus exercise group (group C, n=46). There were no any significant differences among the three groups in age, waist circumference, systolic pressure, diastolic pressure, glucose level, triglycerides and high-density lipoprotein cholesterol concentrations (p>0.05). These individuals accepted relevant continuous intervention for three months. After the intervention, the patients in the diet-control plus exercise group were divided into two groups according to their waist circumference (WC) shorten length,≥7cm group (group C1) and<7cm group(group C2). All the data including WC, TG, HDL-c, glucose, systolic blood pressure and diastolic blood pressure of the patients were tested before and after interventions, and are analyzed and compared the importance of MS components in MS onset.4. SPSS 13.0 version was used in statistic analysis. P<0.05 is considered significant difference.Results:1. MS incidence was 41.3% in male and 8.4% in female in the workers on the job at the authorities in Dezhou. The prevalence of MS presented an increased trend with age. Enlarged WC (≥90 cm) had a increased prevalence (62% in male and 48% in female) and a positive-correlation with elevated either blood pressure or triglyceride.2. Blood pressure, blood glucose and triglyceride levels were significantly higher in the obese group (WC≥90cm) at every age stage (every ten years) than those in the non-obese group (P<0.05).3. The elevated WC, blood pressure, triglyceride, HOMA-IR and OGTT were ameliorated significantly in group C after intervention (P<0.05). The subjects in group C1 (WC shorten more than 7cm) were younger than that in group C2 (WC shorten less than 7cm) (43±10.6 vs.48±9.7 years, P< 0.05). The levels of blood pressure, triglyceride, HOMA-IR and OGTT in group C1 were significantly improved than those in group C2(P<0.05).Conclusion: 1. The incidence of MS of the on-the-job workers in the authorities in Dezhou City is 41.3% in males and 8.4% in females, and the incidence of males are much higher than that of females. It maybe relate to their unhealthy work and life styles.2. WC increasing had strong relationship with high diastolic blood pressure in Han male MS patients in Dezhou.3. Enlarged WC maybe one of the clinical initiation factors of MS in Han male MS patients in Dezhou.4. Diet control plus exercise intervention can help control WC and guide to prevent from MS in Han male people in Dezhou. And early intervention has a better effect.
