Objectives:1. To observe the effect on cognitive function of the mental retardated children by acupuncturing Chang qiang point and to research the clinical significance of selecting the point from a distance to treat the disease.2. The study of expression of upstream activator BDNF,ERK1/2, downstream microtubule associated protein 1B and synaptophysin illustrates the mechanisms of plasticity on Fmrl knocking out mice synapse by puncturing, and provides theoretical basis for mechanisms of modifying cognitive function of mental retardated children by puncturing Chang qiang point.Methods:1. Clinical ResearchThere are 60 cases meeting the inclusion criteria. The 60 cases were randomly divided into changqiang group(30 cases) and Scalp group(30 cases) according to the order of clinical time. Between the two group, Patients in the two groups had 12-week treatment with twice a week. During the 12 week, there were 2 cases off in changqiang group and 3 cases off in Scalp group, so 55 cases are valid. Patients were evaluated respectively by the Wechsler Intelligence Scale and Living Scale of the infant-middle school children before the treatment, 6 weeks after the treatment, and 12 weeks after the treatment. Patients' intelligence and social adaptation are totally evaluated by comparing their different kinds of intelligences in Wechsler Intelligence Scale and different kinds of abilities in social life. At the same time, the improvement of patients'intelligence and social adaptation before and after the treatment were compared by and analyzed by Pearson correlation analysis in different ages children.2. Experiments ResearchWe Select the age of 28-day FMR1 knockout mice (KO) and wild type (WT) mice, each were 30, which were divided into 6 groups:KO Chang Qiang group,KO Non-Point group,KO cntrol group,WT Chang Qiang group,WT Non-Point group,WT control group, each are 10. Continuous intervention on them lasted 6 days, and the brains were removed on the 7th day, and detected the expression of BDNF, ERK2, MAP-1B and the SYN protein by immunohistochemical methods in mouse hippocampus region. BDNF,ERK2,MAP-1B are observed under a microscope. The percentage of positive cells whose cell membrane, nucleus, cytoplasm appeared brown particles are regarded as the key parameter to judge the results. S YN was measured by the average light density values of specific immune reaction products to judge the results.Results:1. Clinical ResultWechsler Intelligence assessment showed that:there were no significant difference (P> 0.05) in the children's VIQ, PIQ and FIQ before the treatment, which are comparable. Comparing in the group:in the Chang Qiang group, there were significant difference (P <0.05) in groups after 12 weeks of treatment compared with before treatment,6 weeks after treatment. In Scalp acupuncture group, there were significant difference in groups (P<0.05) after 12 weeks of treatment compared with before treatment,6 weeks after treatment. Compared between the groups:after 12-week treatment, there were no significant difference between the Chang Qiang group and the scalp group (P>0.05).The results of Living Scale of the infant-middle school children indicated that:there were no significant difference (P>0.05) between the two groups before treatment of the children with SH, L, O, C, S, SD and S-M score, which are comparable. Comparing in the group:in the Chang Qiang group,6 weeks after treatment, there were no significant difference in groups with the SH and SD (P>0.05) after 12 weeks of treatment compared with before treatment. There are significant difference in groups with the L, O, C, S and SM(P <0.05). In Scalp acupuncture group, there were no significant difference in groups with the SH and SD (P>0.05) after 12 weeks of treatment compared with before treatment,6 weeks after treatment. there are significant difference in groups with the L, O, C, S and SM(P <0.05). Comparing between the groups:after 12-weeks treatment, the Chang Qiang group compared with the scalp group were no significant difference (P>0.05).The treatment about IQ and SM among children with different ages shows its rising trend.Correlation analysis of Pearson:The age had negative correlation with Living Scale of the infant-middle school children and WAIS-RC (P<0.01).2. Experimental ResultResults of BDNF protein indicates that:The expression of BDNF positive cells is mainly in the part of nucleus and membrane. The expression of BDNF of FMR1 knockout mice in hippocampus were lower than Wild-type mice, there are significant difference (P<0.05) in statistical comparison. The expression of BDNF FMR1 knockout mice in Chang Qiang group was significantly higher than Non-acupoint group and the control group, and there are statistically significant difference (P<0.05); the expression of BDNF in each Wild-type mice group was not significant (P>0.05).Results of ERK2 protein shows that:The expression of ERK2 positive cells is mainly in the membrane. The expression of ERK2 of FMR1 knockout mice in hippocampus were lower than Wild-type mice. In statistical comparison, there are significant difference (P<0.05). The expression of ERK2 in each FMR1 knockout mice group was not significant (P>0.05), the expression of ERK2 in each Wild-type mice group was no significant (P>0.05).Results of MAP-1B protein shows that:The expression of MAP-1B positive cells is mainly in the membrane. The expression of MAP-1B of FMR1 knockout mice in hippocampus were lower than Wild-type mice. In statistical comparison, there are significant difference (P<0.05), the expression of MAP-1B in each FMR1 knockout mice group was no significant (P>0.05), the expression of MAP-IB in each Wild-type mice group was not significant (P>0.05).Results of SYN protein shows that:The expression of SYN positive cells is mainly in the cytoplasm and intercellular space. The expression of SYN of FMR1 knockout mice in hippocampus were lower than Wild-type mice. In statistical comparison, there are significant differences (P<0.05), the expression of SYN FMR1 knockout mice in Chang Qiang group was significantly higher than Non-acupoint group and the control group, there are statistically significant difference (P<0.05), the expression of SYN in each Wild-type mice group was not significant (P>0.05).Conclusion:1. Acupuncturing the Chang qiang point can effectively improve the capability of children with MR in the VIQ, PIQ, FIQ and L, O, C, S,SM. Moreover,the effect are similar with acupuncturing the Chang qiang and scaple. So there is not only to select scalp in local, but also to select the point from distance, for example Chang qiang point.2. To the children with mental retardation, the younger they are treated the better they can be recovered.3. The expression of BDNF, ERK2, MAP-1B and SYN in brain were regulated positively by FMRP in normal circumstances. The acupuncturing the point of Chang Qiang may improve the hippocampus BDNF and SYN of FMR1 knock out mice.
