Background and Objective: It has been commonly recognized that insulin resistance(IR) and dysfunction of pancreaticβ-cells are two major causes and features of the development of type 2 diabetes mellitus. Recently, with the development of research on pathogenesy of type 2 diabetes mellitus, the concept of pancreatic islet cells insulin resistance has been proposed gradually. Following the demonstration of the presence of the insulin receptor (IRc) and its main substrates, insulin receptor substrate (IRS)-1 and IRS-2, as well as downstream signal transduction in pancreatic islets, several reports have provided evidence for that islet cells is not only the insulin-producing cell but also the targets of insulin. Like the periphery tissues, impairment of any component of insulin signaling in islets cells could cause insulin resistance, i.e."islet cells insulin resistance". Insulin resistance in islets cell directly cause the impairment of islets. This correlation in the level of islets between insulin resistance and islets cells dysfunction may be an important link of pathogenesy of type 2 diabetes. Islet cells insulin resistance may be an key point in prevention and cure of type 2 diabetes, especially in the early stage. However, the study on this question just began years ago. At present, the initial knowledge on molecular biologic mechanism of islet cells insulin resistance just obtain from study on related animal model. There are still a lot of work need to do.Based on the systematical review of materials about clinical manifestation, etiology and pathogenesis of diabetes and insulin resistance, we consider that gan wei yu re symptom is the major pattern of syndrome in Traditional Chinese Medicine. According to this, the principle of Chinese medical treatment is to relieve the heat in liver and stomach, to regulate the Qi in Zhong Jiao. We make up the prescription, kai yu qing wei granule, which come from Da cai hu decoction of《Shang Han Lun》witten by Zhang Zhongjing. The data from previous clinical and experimental research on this prescription had provided evidence for relieving insulin resistance. And it can can improve the insulin sensitivity in insulin resistance rats by relieving intracellular triglyceride content of skeletal muscle and liver, and increasing the number and bonding force of insulin receptor in tissues, and affecting insulin signal transduction. Recent data showed that kai yu qing wei granule could improve diabetic patient insulin secretion and restore islets function, but the mechanism of effect was unknown.
