| The history and advances of(the studies about) reduction mammaplasty are reviewed. The posterior areterial supply of the breast was studied under operating microscope on 18 sides of adult female cadever. The principles of the glandular pedicle design in reduction mammaplasty were summarized and a new operative technique was established according to the results of the anatomic study. The clinical effects were observed (with clinical application and comparision).We have following findings in the anatomic study,1. There are rich posterior arterial supply to the breast parenchyme,originated mainly from the internal thoracic artery and lateral thoracic artery. They have) anastomoses behind the gland and form the posterior mammary vascular plexus. The distribution of the posterior arterial perforators are mostly dense in inferior-medial qudrant.2. One posterior perforating artery, of a larger diameter than the others, nourishes the nipple-areola. It passes through the center of the breast, called " deep papillary artery",with type V and Y.3. There are superficial mammary arterial perforators entering into the mammary parenchyme. They have anastomoses with the posterior mammary arterial perforators and send branches to the nipple-areola.4.
