ObjectiveTo observe the effect of acupoint injection on hearT-qi deficiency syndrome of chronic heart failure (CHF) as well as investigate the influences of different medicines and different doses of identical medicine on the effect.MethodsThis research contains three experiments, all conducted with SPF level male Wistar rats.①Establish the rat model of hearT-qi deficiency syndrome of CHF. Set two groups by randomizing:the blank control group and the model group. Establish the model by forced burden swimming under restricted feeding and drench them with large doses of propranlolum. Check the model and evaluate the molding method by observing rats’symptoms and signs as well as testing indicators as follow:detecte the concentration of atrial natriuretic peptide (ANP) and cyclic adenosine monophosphate (cAMP) in rat blood plasma by ELISA; determine the activity of superoxide dismutase (SOD) in rats’blood plasma through xanthine oxidase method; check the pathologic changes of myocardial tissue by HE stain; check the ultrastructure of myocardial cells by transmission electron microscope; apply RM6240biological information collection and processing system to detect cardiac hemodynamic indicators.②Comparative study on the effect of acupoint injection with different kinds of medicines in treating hearT-qi deficiency syndrome of CHF. Modelling by the method of part one. Set6groups with8rats in each group by random grouping method, such as the blank control group, the model group and4treatment groups (Shenqi group, Danshen group, Jigan group, Distilled water group) which are treated by Zusanli point injection with different medicines that respectively are Shenqi-fuzheng injection, Danshen injection, Jigan injection, Distilled water. Do no processing after molding except give common feed and water everyday for the model group. Give common feed and water everyday only for the blank control group. After 10days’continuous treatment, observe the effects of acupoint injection on hearT-qi deficiency of CHF and the influence of different medicine on the effects by rats’ symptoms and signs as well as indicators refered in part one.③Comparative study on the effect of acupoint injection with different doses of identical medicine in treating hearT-qi deficiency syndrome of CHF. Modelling by the method of part one. Set7groups with8rats in each group by random grouping method, such as the blank control group, the model group and5treatment groups (0.05ml group,0.10ml group,0.15ml group,0.20ml group,0.25ml group) which accept different doses of Shen-fuzheng injection that respectively are0.05ml,0.10ml,0.15ml,0.20ml and0.25ml from ST36acupoints. Do no processing after molding except give common feed and water everyday for the model group. Give common feed and water everyday only for the blank control group. Continue the treatments for10days, and then analyze the dose-effect relationship between the medicine dosage administrated and the effects of acupoint injeciton on heart qi deficency syndrome of CHF by rats’ symptoms and signs as well as indicators refered in part one.Results①Establish the rat model of hearT-qi deficiency syndrome of CHF:after modeling, the model group showed moderate or severe hearT-qi deficiency syndrome. Compared with the blank control group, the model group manifested by the concentration of ANP increased; the concentration of cAMP decreased; the activity of SOD reduced; LVSP, HR and±p/dtmax all decreased; LVEDP, T-DP, T-SP and T-dp/dtmax all increased with statistical differences (p<0.05or p<0.01orp<0.001); pathological changes of myocardial tissue such as myocardial cells swelling, disorder arrangement, inflammatory cell infiltration, et al.; pathological changes of the ultrastructure of myocardial cells such as disorder arrangement, fracture, dissolve of myofilament, mitochondrial crista obscured, partly dissolved, vacuolation.②Comparative study on the effect of acupoint injection with different kinds of medicines in treating hearT-qi deficiency syndrome of CHF:The model group significantly inferior to the blank control group in syndrome, ANP, cAMP, SOD and hemodynamics indicators (p<0.05orp<0.01orp<0.001), with significant pathological changes of the ultrastructure of myocardial cells. Compared with the model group, all treatment groups menifested by hearT-qi deficiency syndrome relieved(p<0.05orp<0.01); ANP, cAMP, SOD and hemodynamics all improved (p<0.05or p<0.01); pathological changes of the ultrastructure of myocardial cells reduced, among them, Shenqi group is the most close to normal. All medicine groups better than Distilled water group. Shenqi group is better than Jigan group and Danshen group in syndrome score, ANP, cAMP,±dp/dtmax, T-DP and T-SP (p<0.05). There is no statistically difference between Danshen group and Jigan group.③Comparative study on the effect of acupoint injection with different doses of identical medicine in treating hearT-qi deficiency syndrome of CHF:The model group significantly inferior to the blank control group in syndrome, ANP, cAMP, SOD and hemodynamics indicators (p<0.05or p<0.01or p<0.001), with significant pathological changes of myocardial tissue. Compared with the model group, all treatment groups menifested by hearT-qi deficiency syndrome relieved (p<0.05or p<0.01); ANP, cAMP, SOD and hemodynamics all improved (p<0.05orp<0.01); pathological changes of myocardial tissue reduced. The curative effect of acupoint injection on hearT-qi deficiency of CHF keep positive relationship with the dosage ranged from0.05ml to0.20ml.Conclusions①’forced burden swimming under restricted feeding and drenched with large doses of propranlolum’ is a feasible method for establishing hearT-qi deficiency syndrome of CHF rat model.②Acupoint injection can improve the cardiac function of rats with hearT-qi deficiency syndrome of CHF. Manifested in improving rats’ cardiac hemodynamic condition, repairing the damaged myocardial tissue and ultrastructure of myocardial cells, reducing the blood plasma concentration of ANP, increasing the blood plasma concentration of cAMP and the activity of SOD, relieving the hearT-qi deficiency syndrome of rats.③Acupoint injection with medicine administrated according to syndrome differentiation will make the effects better.When treat hearT-qi deficiency syndrome of CHF with acupoint injection, to inject medicine can create better effect than non-pharmaceutical injection; Shenqi-fuzheng injection which is good at tonifying qi is better than curculation-activating Danshen injection and energy metabolism-improving Jigan injection.④Dose-effect relationship exists between the medicine dosage administrated and the effect of acupoint injection in treating hearT-qi deficiency syndrome of CHF. The appropriate dosage of medicine will contribute to enhancing the curative effects of acupoint injection.
