| Helicobacter pylori has played a critical role in many gastrointestinal diseases since it was discovered. It is thought that there are quite a few determinants lead to the pathogenesis of Hp-related gastrointestinal diseases, from adhesion of bacterial strain and damage of the virulence factor for gastric mucosa, the influence of social environment to the host susceptibility and reactivity. Human leukocyte antigen(HLA)gene complex system is most strongly associated with the diseases. It determines the individual differences of the host. Many studies have suggested that HLA gene may be important in determining the susceptibility and clinical outcome of Hp infection.This study aimed to investigate the relationship between HLA-â…¡gene polymorphism and Helicobacter pylori infection in Lanzhou population by case-control study. And a meta- analysis was performed in order to explore the association between HLA-â…¡gene polymorphism and Helicobacter pylori infection according to the existing researches.1 Association between HLA- â…¡ gene polymorphism and Helicobacter pylori infection in Lanzhou1.1 HLA-â…¡ gene may be important in determining the susceptibility and clinical outcome of Hp infection. There may be resistant gene or susceptible gene of Hp infection in HLA- â…¡ alleles. This study aimed to explore the association between HLA- â…¡ gene polymorphism and Helicobacter pylori infection in Lanzhou population.1.2 A case-control association study in patients of Hp-related gastrointestinal diseases(n = 144) and healthy controls(n = 144) of Lanzhou population was conducted. PCR-SBT was adopted to perform HLA-DQB1ã€HLA-DRB1 genotyping.1.3 In Lanzhou population, the statistically significant protective gene in Hp infection involved HLA-DQB1﹡0302 and HLA-DRB1﹡1501 alleles(P=0.041,OR = 0.589, 95% CI: 0.353-0.983;P=0.009, OR = 0.476, 95% CI: 0.271-0.839respectively); HLA- DQB1﹡0301 acted as the statistically significant susceptible gene in Hp infection( P= 0.005, OR = 1.985, 95% CI: 1.228-3.208). And no statistically significant difference(P>0.05)in the association between the rest of HLA-â…¡alleles and Hp infection.1.4 In Lanzhou the protective gene in Hp infection involved HLA-DQB1﹡0302and HLA-DRB1﹡1501 alleles and HLA- DQB1﹡0301 alleles was associated with a increased risk of infection of Hp.2 Association between HLA- â…¡ gene polymorphism and Helicobacter pylori infection: a meta-analysis2.1 To perform a meta-analysis to explore the relationship between HLA-â…¡gene polymorphism and host susceptibility to Hp infection.2.2 Relevant cohort studies, case-control studies and cross-sectional studies were identified by searching Cochrane Library, Pub Med, EMBASE, Web of Science and CBM up to July 2014. The data were extracted and methodological quality of the studies were evaluated. Rev Man 5.0 software was used to perform statistical analysis.2.3 In Asian population, HLA-DQB1 ﹡ 0303 acted as the protective gene in Hp infection(statistically significant pooled OR =0.54)and the susceptible genes in Hp infection involved HLA-DQB1 ﹡ 0401, HLA-DQA1 ﹡ 0103 and HLA-DQA1 ﹡0301(statistically significant pooled OR and 95%CI were 3.34(1.93,5.77),1.64(1.16,2.33) and 2.03(1.20,3.44) respectively). No statistically significant difference between DQB1﹡0303, HLA-DQA1 ﹡0103 and DQA1 ﹡0301 and Hp infection in European population(P>0.05). And no statistically significant difference(P>0.05)in the overall effect of the association between the rest of HLA-â…¡alleles and Hp infection.2.4 In Asian population, the protective gene HLA-DQB1﹡0303 and the susceptible genes HLA-DQB1 ﹡ 0401, HLA-DQA1 ﹡ 0103 and HLA-DQA1 ﹡ 0301 in Hp infection were established by meta-analysis. And there was no HLA- â…¡ allele was found to associate with Hp infection among European population. |
