| Objective:To observe cartilage MRI evaluation of Zhitongjiangu decoction on knee osteoarthritis and correlation analysis with the NF-κB signaling pathway,provide experimental basis for clinical application.Method:90 rabbits were randomly divided into normal control group, model control group, positive drug group, Zhitongjiangu low dose group, Zhitongjiangu high dose group, each group with 18 rats in each, the preparation of rabbit model of KOA papain intra-articular injection of and model verification is successful, Zhitongjiangu low dose group, high dose group, to give the corresponding Zhitongjiangu recipe intragastric administration, and the positive drug group were given hydrochloride glucose tablets orally, normal control group, model control group were intragastrically administrated with the same dose of normal saline. In front of the experimental intervention, the intervention of the 4 weeks,8 weeks were measured of the knee flexion measurement and examination of knee joint MRI. After the rabbits right knee joint cavity were open, gross observation and light microscope, micro morphological observation of articular cartilage and synovial membrane, and immunohistochemical detection of MMP-1 and TNF-a expression and RT qPCR cartilage IKK a mRNA and NF-κB mRNAexpression and MRI evaluation of correlation with the NF-κB signaling pathway analysis.Results:① The use of the joint cavity injection method can be successfully prepared rabbit KOA, the preparation of KOA animal model of pathological changes with human KOA similar.② the former experimental intervention, compared with the normal control group, model control group, positive drug group, Zhitongjiangu low dose group, Zhitongjiangu high dose group at the knee flexion, knee joint cartilage MRI grading of articular cartilage and Mankin’s score and synovial tissue score, MMP-1 and TNF-a expression, cartilage IKK a mRNA and NF-κB mRNA were increased, the difference is statistically significant (P<0.05), model control group, positive drug group, Zhitongjiangu low dose group, Zhitongjiangu high dose group four groups, the difference was not statistically significant (P> 0.05).③ The experimental intervention for 4 weeks, compared with the normal control group, model control group, positive drug group, Zhitongjiangu low dose group, Zhitongjiangu high dose group in knee flexion, articular cartilage Mankin’s score and synovial tissue score, MMP-1 and TNF-a expression, cartilage IKK a mRNA and NF-κB were increased, the difference is statistically significant (P<0.05). Compared with the model control group, positive drug group, Zhitongjiangu low dose group, Zhitongjiangu high dose group in the articular cartilage of the knee MRI grading difference no statistical significance (P> 0.05), but the rest of the indicators have improvement difference was statistically significant (P<0.05). There was no significant difference in the positive drug group, Zhitongjiangu low dose group, and Zhitongjiangu high dose group at fourth weeks (P> 0.05), but the indexes were improved before the intervention (P<0.05).④ Experimental intervention of 8 weeks, compared with the normal control group, model control group, positive drug group, Zhitongjiangu low dose group in knee flexion, knee joint cartilage MRI grading of articular cartilage and Mankin’s score and synovial tissue score, MMP-1 and TNF-a expression, cartilage IKK a mRNA and NF-κB mRNA were increased, the difference is statistically significant (P<0.05), Zhitongjiangu high dose group of knee joint flexion and knee articular cartilage MRI grading, cartilage IKK a mRNA and NF-κB expression returned to normal level (P>0.05) Compared with the model control group, positive drug group, Zhitongjiangu low dose group, Zhitongjiangu high dose group in the above indexes were improved significantly (P<0.05), compared with the intervention group before, the difference is statistically significant (P<0.05). In the Zhitongjiangu high dose group, the knee flexion activity, cartilage IKK a mRNA and NF-κB mRNA were significantly improved(P<0.05) compared with the positive drug group and the Zhitongjiangu low dose group. MRI evaluation and correlation analysis of the NF-nuclear factor kappa B signal pathway in the treatment of osteoarthritis of rabbit knee osteoarthritis were found, and the two were linear positive correlation (P<0.05).Conclusion:①By papain intra-articular injection can be successfully prepared rabbit koa model, the preparation of KOA model pathological change similar fan KOA and the animal model of NF kappa B signaling pathways exist state of activation and the expression of MMP-1, TNF-a, IKK a mRNA and NF-κB mRNA increased significantly.②In rabbit KOA model, the expression of MMP-1, TNF-a, IKK a mRNA,NF-KB mRNA, MRI, and were increased, the cartilage damage grade and pathological score increased, and the three were positively correlated.③glucosamine hydrochloride tablets on koa has therapeutic effect and mechanism of action may be by inhibiting IKK a mRNA and NF-κB mRNA expression, reduced the expression of downstream products of MMP-1, TNF-a, inhibit inflammatory response and protection of cartilage cells.④ Zhitongjiangu decoction for KOA model has therapeutic effect, the mechanism may through inhibiting the KOA cartilage NF-κB signaling pathway and by inhibiting the expression of IKK a mRNA and NF-κB mRNA and reduce downstream products of MMP-1, TNF-a expression, inhibit inflammatory response and protection of cartilage cells.⑤Knee MRI could diagnose KOA cartilage injury and classification, and it could reflect KOA drugs on the repair of cartilage intuitively and non-invasively, also it orrelate with cartilage pathology score and NF-κB signaling pathway. |
