Professor Jin Bohua's Academic Thoughts And Clinical Experience And Jin's Acupuncture Method For The Treatment Of Ankylosing Spondylitis With Kidney Deficiency And Cold Syndrome

This article is a relatively comprehensive summary of Prof. Jin Bohua’s academic thought and clinical experience from my three years’study as an assistant to Prof. Jin. It includes four aspects:source of Prof. Jin’s academic thought; outline of Prof. Jin’s academic thought; experience in diagnosis and treatment; and clinical research of treating ankylosing spondylitis renal deficiency and cold syndrome with warming needling through moxibustion on governor vessel.1. Source of Prof. Jin Bohua’s Academic ThoughtIt includes the following contens in terms of the Prof. Jin Bohua’s Academic Thought formation:(1) Joined the army to study medicine and participated in the War to Resist U.S. Aggression and Aid Korea:studied in Bethune Medical School of North China Military and Political University, rescued the wounded in battle, which laid a solid foundation in western medicine; (2) Nurtured and influenced by famous masters of medicine on her road to traditional Chinese medicine:Through studies in the Out-patient Department of The No.2 Traditional Chinese Medicine of China Academy of Chinese Medical Sciences and Beijing Hospital of TCM, absorbed essence of the academic achievements of famous masters of internal medicine, such as "Golden Needle" Leting Wang, Puren He, Shouren Xia and others; (3) Intensive and extensive reading and research on ancient Chinese medical books and records demonstrated her unique style:completed junior college studies on classical Chinese medicine and four classics, namely, "Canon of Internal Medicine", "Treatise on Febrile Diseases", "Great Compendium of Acupuncture and Moxibustion" and others, formed her own unique style of thinking on clinical diagnosis and treatment and developed her own Jin Style Needle Operation skills; (4) Combined Chinese with western, and overcame difficulties:hosted "The 8th Five-Plan on Tackling Key Problems-Traditional Chinese Medicine Preparation on’Zhuifengsu’Acupoint Solution Injection and Acupuncture and Moxibustion Treatment of Rheumatoid Arthritis" research organized by Beijing Municipal Science and Technology Commission, and achieved a national patent. Her project research report and academic papers won the "Scientific and Technical Achievement Award of Beijing Municipal Health Bureau" and the World Traditional Medicine "Vitality Cup" award.2. Outline of Prof. Jin Bohua’s Academic ThoughtCombining Prof. Jin’s monographs, such as "Succinct of Prof. Jin’s Acupuncture and Moxibustion Clinical Practice", "Jin Bohua’s Collection on Poliomyelitis Testing and Treatment", "Jin Bohua-Heroine in Acuputure and Moxibustion" and others, based on Prof. Jin Bohua’s acupunctural clinical theory and insight of clinical practical, by deepening and sublimating Prof. Jin’s academic point of views, summarized creatively the "Gist of Jin Style Acupuncture Methods" which consists of the following six points:"Treatment on the root of disease, most of diseases can be traced to the stagnation of qi", "unique diagnostic methods, detailed observation","regulation as key, heart and kidney as foundation", "combine acupuncture and medicine for mutual supplementation", "make the best use of single acupoint, and the effect is swift", and "carry forward the needling methods, develop a style of its own".2.1 In clinical treatment, with the theory of "The Internal Canon of Medicine" as guide, we must strictly obey the overall traditional Chinese medicinal concept and the basic approach of syndrome differential treatment to cure the root of the disease, emphasize that, for difficult and miscellaneous diseases, we must trace their roots, believing that various modern diseases are caused by the stagnation of qi.2.2 Unique diagnostic method, paying attention to the meridian and local diagnosis and observation. For instance, the stroke patients, observe the vascular changes on both sides of the sublingual frenum, make a preliminary judgment on whether the patients belong to hemorrhage or infarction; for patients with facial paralysis, judge the severity of the disease and its outcome through the Yifeng(SJ17) acupoint pressing pain reaction.2.3 Advocate that for all kinds of diseases, priority shall be given to "regulation" regulate the qi mechanism, regulate the internal organs, regulate Yin and Yang, regulate the blood, and regulate the meridians, so as to achieve the balance of the meridians’ yin and yang for curing the disease. Clinical diagnostic discrimination emphasizes the heart and kidney: "heart, the king of all organs, the source of spirit", principal of the vital organs of the human body; "kidney, receptor and storage of essence of the vital organs of the human body," the congenital foundation. Make good use of Neiguan (PC6), Danzhong(RN17), Jiuwei(RN15) to regulate the mind, and Shenshu(HT7), Mingmen(DU4), and Taixi(KI3) to regulate the kidney essence.2.4 Pay attention to the combination of acupuncture with drug, advocate the combined use of acupuncture with medicine. She points out that an acupuncture physician must make a collaborative use of acupuncture and traditional Chinese medicine with medicine focusing on the six-meridian pattern dialectics and acupuncture focusing on main and collateral meridian pattern dialectics. The combination of multiple acupuncture methods have achieved significant effects on the clinical treatment of thoracic obstruction, hair loss, intractable constipation and others.2.5 Adept at single acupoint treatment of disease, with unique experience in clinical acupoint selection. For clinical treatment, she often uses the Quchi(L111) acupoint for treatment of hypochondriac pain, stomach pain; Taixi(KI3) acupoint for treatment of dry throat, nephrasthenia headache; Shonzhenzugen acupoint for treatment of heel pain; and the flexible application of Xiajngming etc. for symptomatic treatment.2.6 Conducted research meticulously on the needling techniques recorded in the Internal Canon of Medicine, and created special needling methods, such as Liuci, Chilunci, Paici, Meihuaci and other needling methods. For clinical treatment, acupoints are often selected from far end so that acupoints from lower part of the body are often selected for sickness or disease of the upper part of the body, or acupoints from upper part of the body are often selected for sickness of the lower part of the body, make sure that the needling sensation tends to the aching part of the body; imporved the fire burning methods for treatment of diseases, such as paralysis, cold-dampness and arthralgia spasm pain, cold limbs, and stomach cold and other syndrome. In combination with the ancient acupuncture and modern blood stasis pathogenesis theory, skilled in blood-letting puncture for clinical treatment, which is widely applied in all kinds of diseases, such as erysipelas, eczema, gout, rheumatoid arthritis, headache, facial paralysis, cervical spondylosis and other diseases.3. Prof. Jin’s Experience in Diagnosis and Treatment;This study combines and collects the records of Prof. Jin Bohua’s clinical medical cases, analyzes her unique diagnostic treatment methods and academic characteristics based on the academic thought of the "Gist of Jin Style Acupuncture Methods". Starting from the valuable experience of Prof. Jin in her acupuncture treatment of rheumatoid arthritis, stroke, paralysis, facial paralysis, constipation, ankylosing spondylitis, depression and other difficult and miscellaneous diseases, and through her unique etiology and pathogenesis, diagnosis and treatment key points, as well as typical case analysis, give a comprehensive introduction to her diagnosis and treatment experience.4. Clinical Research of Warming Needle Moxibustion Huatuo Spine Points to Treat Ankylosing SpondylitisOn the basis of the clinical experience of Prof. Jin’s treatment of ankylosing spondylitis, by applying the clinical observation of patients through warming needle moxibustion Huatuo spine points to treat ankylosing spondylitis.Objective First, to evaluate the clinical therapeutic effects of Jin’s warming needling through moxibustion on Huatuo spine governor vessel for the treatment of ankylosing spondylitis renal deficiency and cold syndrome. Second, to explore the differences of the therapeutic effects of acupoint selection method for the treatment of ankylosing spondylitis with Huatuo’s spine points.Method This was a randomized, by contrast and single blind study method, and 63 patients with ankylosing spondylitis renal deficiency and cold syndrome were randomized into a treatment group and a contrast group with 32 and 31 patients respectively (at the ratio of 1 to 1). In the treatment group, it was applied Prof. Jin’s Huatuo spine points selection method of 1 cun lateral to the depression below the spinous process, in the contrast group, it was applied conventional Huatuo spine points selection method of 0.5 cun lateral to the depression below the spinous process, for three months totally. The occipital to wall distance, activity of thoracic, Schorber test and spinal pain VAS score during the baseline and three months treatment were recorded, and ASAS20, BASDAI index etc were analysed to evaluate the clinical therapeutic effects and safety of warming needling through moxibustion on Huatuo spine governor vessel for the treatment of ankylosing spondylitis renal deficiency and cold syndrome.Result Through statistical analysis, it shows the overall effective rate, the ASAS20 improvement and BASDAI score comparison of the treatment group are better than those of the contrast group. The spinal pain VAS score, activity of thoracic score, occipital to wall distance score and Schorber test score differences of the two groups before and after the treatment are of statistics significance. There were significant differences in spinal pain VAS score, activity of thoracic score, occipital distance score, Schorber test score between the two groups. During the treatment, the laboratory parameters show that the treatment group experienced significant differences in ESR, CRP, while the contrast group experienced no significant difference in ESR, CRP, and there was no significant difference in ESR, CRP between the two groups on the baseline.Conclusion Based on Jin Bohua’s acupuncture academic point of view and acupuncture clinical experience, the randomized contrast research was conducted. Both Prof. Jin’s warming needling through moxibustion on Huatuo spine governor vessel and conventional acupoint selection method were effective for the treatment of ankylosing spondylitis renal deficiency and cold syndrome with. Compared with conventional acupoint selection method, Prof. Jin’s therapy can more effectively improve the BASDAI index, ASAS20 and other clinical therapeutic effect, and the clinical indicators such as spinal pain VAS score, activity of thoracic score, occipital to wall distance score are of statistics significance. The effect of treatment group is better than that of contrast group. The laboratory parameters show that the treatment group experienced significant differences in ESR and CRP and differences are of statistics significance.Based on analysis of the Huatuo spine points of ancient and modern literature methods and Jin’s clinical acupoint selection, the treatment of ankylosing spondylitis is conducted by Huatuo spine points of 1 cun lateral to the depression below the spinous process, the deep-needling method lies in the needle inserting obliquely to form on angle of 45 degree to spinous process with the skin. This method provides acupoint selection evidence for the treatment of deep-needling Huatuo spine points for paraplegia, cervical and lumbar intervertebral disc herniation, spinal stenosis and other stubborn miscellaneous diseases, and "focal arrival of acu-esthesia", and improving clinical curative effect.
