| objectiveBy using the method of literature mining,this paper explored the distribution law of Qi stagnation and blood stasis syndrome,and formulated evaluation methods of curative effect.Under the guidance of dialectical principle of "treating different diseases with the same treatment",through the randomized controlled trial of Danhong Huayu Oral Liquid on different diseases with the same syndrome,objectively collected and evaluated the tongue、 pulse and fundus images of patients with Qi stagnation and blood stasis syndrome,analysised the characteristics and internal correlation of Qi stagnation and blood stasis syndromes,and evaluated the feasibility of the method ’s application to objectively study of traditional Chinese medicine syndromes.Method1.Data mining: The frequency of symptoms and signs in published literatures on Qi stagnation and blood stasis was analyzed;the frequency of diseases and syndromes of pain syndromes in traditional Chinese and Western medicine was analyzed;and analyzed the relevance of the regularity of clinical medication combination of primary headache,primary dysmenorrhea and retinal meridian obstruction;Metaanalysis of the curative effect of Xuefu Zhuyu Decoction on primary headache and primary dysmenorrhea was carried out and the research programs was formulated on the basis of literature research.2.Study on objective evaluation of Qi stagnation and blood stasis syndrome: A total of 120 clinical subjects of 3 diseases were recruited from 6 third-class A hospitals in China.The curative effect of the same drug intervention on diseases with the same syndrome was observed,and the changes of tongue 、 pulse 、 fundus image and the internal correlation between primary and secondary symptoms were evaluated.The dynamic changes of TCM syndrome score,VAS score,frequency and number of the pain before and after treatment were also observed.Result1.Data mining:(1)The results of literature analysis showed that Qi stagnation and blood stasis syndrome accounted for the largest proportion of all types of pain syndrome,suggested that pain syndrome is a good carrier for the study of Qi stagnation and blood stasis syndrome.(2)The clinical medication of primary headache,primary dysmenorrhea and retinal vein obstruction of Qi stagnation and blood stasis type is mainly composed of blood-activating drugs,Qi-activating drugs and warming prescriptions.The main prescriptions are Xuefu Zhuyu Decoction and its addition and subtraction prescription,showed that the prescriptions containing "Xuefu Zhuyu Decoction" should be selected for clinical medication of Qi stagnation and blood stasis syndrome.(3)The results of Meta-analysis showed that Xuefu Zhuyu Decoction had a good effect in treating primary headache and dysmenorrhea of Qi stagnation and blood stasis type.Danhong Huayu Oral Liquid had clinical feasibility in the study of primary headache and dysmenorrhea with Qi stagnation and blood stasis type.2.Objective research results:(1)After treatment,the diameter of fundus vein in the experimental group decreased compared with that before treatment,suggested that Danhong Huayu oral liquid may restore the circuitous and dilated diameter of fundus vein in patients with Qi stagnation and blood stasis.(2)By correlation analysis we found that the “fixed Pain-irritable-dark red or purple lumps of menstruationbreast swelling pain”is the most common secondary syndrome combination of Qi stagnation and blood stasis syndrome,“string pulse-pale red tongue-white coating-thin coating”is the most common combination of tongue and pulse signs of Qi stagnation and blood stasis syndrome,which indicated that there was a certain internal correlation between the TCM syndrome of Qi stagnation and blood stasis,and its occurrence and combination showed regularity.(3)The tongue and pulse normalization rates in the experimental group were higher than that in the control group after treatment,suggested that Danhong Huayu oral liquid can improve the objective signs of patients with Qi stagnation and blood stasis more effectively than placebo.Tongue image,pulse image and fundus image may be one of the objective indicators to judge the syndrome of qi stagnation and blood stasis.(4)The disappearance rate of main symptoms in the experimental group was higher than that in the control group,and the disappearance rate of most secondary symptoms was also higher than that in the control group.It suggested that Danhong Huayu Oral Liquid could improve the main symptoms and secondary symptoms related to Qi stagnation and blood stasis,which is more effectively than placebo and had better curative effect on the improvement of clinical symptoms.3.Other indicators:(1)VAS score: The VAS score of the subjects in the primary headache and dysmenorrhea test group decreased significantly from the second course of treatment,and showed a downward trend with the progress of the course of treatment,and the downward trend was more significant than that of the control group.(2)Duration of pain onset: Duration of pain onse of the two groups of primary headache showed a slower downward trend,there was no significant difference between the 1-3 to courses of treatment.The duration of pain onset in the 4 course of treatment was lower than that in the control group.The duration of abdominal pain episodes in the experimental group of primary dysmenorrhea decreased with the progress of treatment,but this tendency was not appeared in the control group.(3)Frequency of pain onset: There was no significant difference in the frequency of headache between the two groups during the 1-3 courses.In the 4 course and follow-up period,the frequency of headache in the experimental group was lower than that in the control group.The frequency of dysmenorrhea onset in both groups declined with the progress of treatment.In the 3 courses of treatment,the frequency of dysmenorrhea in both groups decreased significantly compared with that before treatment.(4)CMSS score: CMSS score of primary dysmenorrhea patients in both groups showed a downward trend with the progress of treatment and it significantly decreased after treatment compared with before.(5)TCM syndrome score: The TCM syndrome score of two groups of primary headache and primary dysmenorrhea were significantly lower than that before treatment from the 2 course of treatment,and showed a downward trend with the progress of treatment.It was suggested that both Danhong Huayu Oral Liquid and placebo can improve the clinical symptoms of primary headache and primary dysmenorrhea of Qi stagnation and blood stasis type.4.Therapeutic indicator: According to the conversion formula of TCM syndrome score,the effective rate of the experimental group was significantly higher than that of the control group,suggested that Danhong Huayu oral liquid had better curative effect than placebo in treating Qi stagnation and blood stasis syndrome.Conclusion1.Data mining showed that there is a certain internal correlation between TCM syndromes of qi stagnation and blood stasis,which has regularity in appearance and combination.The objectivism acquisition of tongue、pulse and retina image technology are conducive to the objective study of syndromes and to discover the regularity and essence of syndromes.2.Based on the similarity of pathogenesis,it is feasible to use the same syndromes drugs to treat different diseases with the same syndromes under the guidance of TCM syndrome differentiation theory "treating different diseases with the same treatment",Moreover,it is helpful to observe the evolution relationship between diseases and to find evidence of the value of common treatment and prevention. |
