| Part one: Study on the Syndrome Properties of Non-surgical Model of SPID Rats Based on the Counterevidence of PrescriptionObjective: Taking drug intervention in SPID model rats with qi stagnation and blood stasis syndrome determined in the early stage of this experimental group,defending the characteristics of its syndromes,and preliminarily discussed the mechanism of the treatment of SPID by Shugan Liqi and Huoxue Huayu method through animal experiments.Methods: Divided the 93 SPID model rats with qi stagnation and blood stasis syndrome into five groups according to the random number table method,,namely,Xiaoyao Shukun Decoction Group(21),Lianbai Decoction Group(21),pelvic cavity.Yantang group(21),Guizhi Fuling group(20),and blank control group(10).The drug solution was administered to the stomach for 14 days.Observed the TCM syndrome scores,gross morphology and local pathological changes before and after administration.The expression of CD3+,CD4+/CD8+,Th17 and Foxp3 in peripheral blood of each group was detected by peripheral blood flow cytometry.The production of immune factors including TNF-α,IL-4,IL-6,IL-10,IL-17,TGF-β1,MMP-2 and VEGF in tissues and serum of rats was detected by enzyme-linked immunosorbent assay.The m RNA transcription levels of all the immune factors and NKG2 D,NKG2A in tissues and serum of rats was detected by real-time quantitative nucleic acid amplification detection system.Results: After drug intervention,the distortion,adhesion and edema condition of the fallopian tube,ovary and pelvic connective tissue of the Xiaoyao Shukun Decoction group were significantly improved.The TCM syndrome scores were significantly decreased,and the blauer score of pelvic connective tissue was significantly lower than that of the blank control group.The inflammation scores of the fallopian tube and pelvic connective tissuesections were significantly lower than those of the other groups(P<0.05 or P< 0.01);the expression of CD3+ and CD4+/CD8+ in peripheral blood of rats was higher than that of the blank control group,the expression rate of Th17 and Foxp3 was decreased,Expression levels of TNF-α,IL-4,IL-6,IL-17,TGF-β1,MMP-2,VEGF and gene transcription levels in oviduct,pelvic connective tissue and serum were decreased,IL-10 expression level and gene transcription level increased,NKG2 D m RNA transcription level decreased,NKG2 A m RNA transcription level increased,compared with other groups were statistically significant(P <0.05 or P <0.01).Conclusions: The majority of TCM syndromes of SPID model rats were qi stagnation and blood stasis.Xiaoyao Shukun Decoction with Shugan Liqi and Huoxue Huayu as the legal cube has obviously improved the TCM syndrome and altered pathological indexes,it is proved that the syndrome of qi stagnation and blood stasis is rationality and scientificity.Filled in the gaps of the syndrome properties research of non-surgical establishment of SPID animal model.Xiaoyao Shukun Decoction can increase the expression of CD3+ and CD4+/CD8+,reduce the expression of Th17 and Foxp3 to regulate and improve the immune function of the body.Decreased the expression of TNF-α,IL-4,IL-6,IL-17,TGF-β1,MMP-2,VEGF and NKG2 D,increased the expression of IL-10 and NKG2 A to inhibite the infiltration of inflammatory cells,reduce the inflammatory damage,inhibits the pathological changes of pelvic connective tissue fibrosis and hyperplasia,prevents and reduces the formation of adhesions,promotes the repair and regeneration of tissues,and thus achieves the purpose of treating SPID.Part two: Effect of Xiaoyao Shukun Decoction on Hemorheology in SPID PatientsObjective: To observe the clinical efficacy and the effect of Xiaoyao Shukun Decoction on hemorheology indexes of SPID patients with chronic pelvic pain(qi stagnation and blood stasis syndrome),and to explore its mechanism.Methods: Selected eighty patients with SPID and divided them randomly into treatment group: forty cases,treated with Xiao Yao Shu Kun Decoction,and control group:forty cases,treated with Kun Fukang capsule.Observed the changing of TCM syndrome,local signs,pain effect and hemorheology indexes of each group after treatment.Results: After treatment,the combined efficacy and pain efficacy of the two groups were significantly different(P<0.01).There were significant differences in the indexes of blood rheology within the group(P<0.01).The high cut whole blood viscosity and hematocrit result were significantly different between the two groups(P<0.01).The lowcut whole blood viscosity,plasma viscosity and blood sedimentation result had different between the two groups(P<0.05).Conclusions: Xiaoyao Shukun Decoction can significantly improve the clinical symptoms and local signs of SPID patients,improve the hemorheology indexes,improve the blood concentration,viscosity,coagulation and stagnation of patients,promote pelvic blood circulation,thereby achieving its therapeutic purposes. |
