| Objective:Xiang thinking is the original thinking of China,and is the main thought of traditional Chinese medicine.Xiang thinking originated from the ancient book Zhouyi.HuangdiNei jing began in the Warring States period,which laid the theoretical foundation for Chinese medicine and the application of Xiang thinking is everywhere in the book.This study aims to explore the connotation and classification of "Xiang",the connotation and characteristics of Xiang thinking,the comparison between Xiang thinking and conceptual thinking,and the application of xiang thinking in traditional Chinese medicine.This research aims to cause the attention of traditional Chinese medicine learner with xiang thinking and return to traditional Chinese medicine thinking.We should widely absorb the research achievement of modern medicine and many disciplines,and apply the Xiang thinking to the cognition of traditional Chinese medicine for tumor angiogenesis,aiming at providing ideas for traditional Chinese medicine clinical.Method:Based on the document research,theoretical research and clinical research,this thesis consults the ancient and modern literature about xiang thinking and discusses the general research status of xiang thinking.By combining with the theory of xiang thinking,we investigate the Chinese medicine theory of tumor angiogenesis,and apply to clinical practice under the guidance of theory.Clinical research is to further expand the research sample after the initial curative effect is obtained in clinical practice.This study selects 60 advanced primary liver cancer patients who were hospitalized from January 2017 to July 2019 as the subjects of this study.They were randomly divided into treatment group(QingHuoTongLuo and Xiaoyao Powder combined with TACE,30 patients)and control group(TACE,30 patients).Before and after treatment,laboratory indicators,efficacy of solid tumors,adverse events,quality of life,TCM symptoms score of the two groups of patients were observed and then compared extensively,survival rate comparison.Result:This thesis discusses the research of xiang thinking,the theory research and clinical application of xiang thinking in tumor angiogenesis,the clinical observation on the effect of prescription for QingHuoTongLuo and Xiaoyao Powder combined with TACE treatment of advanced liver cancer under the application of xiang thinking.The first part is the research of xiang thinking.Firstly,we discuss the connotation and classification of xiang thinking.The meaning of xiang thinking is very rich.The meaning of "Xiang"begins with the meaning of the xiang of elephant,and there are many extensions.as a noun,there are meanings of images,phenomena,symbolic symbols.As a verb,there is a similarity,emulation,and symbolic meaning.The meaning of "Xiang" is described in many works.Such as Zhou Yi,Lao Zi,Zhuang Zi and the HuangdiNei j ing.According the meaning of xiang,the xiang is divided into the following five categories:the xiang of object,image,the xiang of model,comparative state,and xiang.Secondly,the thesis discusses the connotation and application of xiang thinking.Xiang thinking is a unique way of thinking of the ancients.The ancients'understanding of all things in the world is based on the "Xiang".Xiang thinking,as a kind of thinking activity,can be carried out in a step-by-step manner according to certain procedures and steps,and there are also ways to follow it.The author believes that xiang thinking is using the thinking of classification to classify things on the basis of acquiring the "xiang”of nature.Then establish the Xiang model,analyze the relationship between different kinds of things according to the internal relations in the Xiang model,so as to explain the known things and recognize the unknown things.The specific thinking steps are xiang abstracting from viewing,comparative state,setup(use)xiang model,xiang for expression.The Xiang thinking has the characteristics of integrity,functionality,heterogeneity,fluidity,fuzziness,relationship,innovation,etc.Thirdly,the thesis discusses the comparison between Xiang thinking and conceptual thinking.They have essential differences in characteristics,methods and purposes.The relationship between xiang thinking and conceptual thinking contradicts each other as well as complements each other.One of the main differences between Chinese medicine and Western medicine is thinking patterns.TCM focuses on "Xiang" and mainly uses a whole,dynamic,connection view to recognize things.Western medicine focuses on "concepts",mainly uses conceptual thinking,and based on anatomy.Lastly,the thesis discusses the application of Xiang thinking in the traditional Chinese medicine from the basic theory of Chinese medicine,diagnosis of traditional Chinese medicine,Chinese pharmacy,meridian and modern research of TCM.The second part is the theory research and clinical application of xiang thinking in tumor angiogenesis.This study summarizes the etiology and pathogenesis of tumor angiogenesis according to xiang thinking.“cancer toxin”is the initiating factor of tumor angiogenesis;fire evil is the key driving force for tumor angiogenesis;obstruction of collaterals is the material basis of tumor angiogenesis.dispelling "cancer toxin",relieving exterior heat,resolving stasis for dredging collaterals are important therapeutic methods for anti-tumor angiogenesis.And creating QingHuoTongLuo prescription to resist the formation of tumor blood vessels.In this study,we used xiang thinking to compare normal blood vessels to minor fires,tumor angiogenesis to vigorous fire,and anti-tumor angiogenesis-targeted drugs to resemble "fire extinguishing".Therefore,the application of anti-tumor angiogenesis drugs while using traditional Chinese medicine Shenqi Bolus to supplement the minor fire to produce kidney qi can reduce the impact of anti-tumor angiogenesis drugs on some of the body's minor fire,in order to reduce side effects.The combined application of anti-tumor angiogenesis drugs and Chinese medicine QingHuoTongLuo can dispel“cancer toxin",relieving exterior heat,resolving stasis for dredging collaterals,and is expected to have multiple targets against tumor angiogenesis,thereby delaying or even reversing drug resistance.This study also uses xiang thinking to compare the TACE to“fire extinguishing”,while tumor vascular embolization is similar to the close the natural gas supply pipeline at the fire site,and tumor perfusion chemotherapy is similar to injecting fire extinguishing agent into the natural gas supply pipeline of the fire site.The two biggest problems after TACE are to solve postoperative adverse reactions(clean up the fire scene)and tumor angiogenesis(Extinguish the residual fire).Using Xiaoyao Powder to soothe liver and strengthen spleen can reduce the impact of TACE on the human body and reduce adverse reactions after TACE.Combined use of traditional Chinese medicine QingHuoTongLuo after TACE in clinical practice can dispel“cancer toxin”,relieving exterior heat,resolving stasis for dredging collaterals,as well as anti-tumor angiogenesis,to prevent recurrence and metastasis after TACE.And appling the TCM theory of tumor angiogenesis under the guidance of xiang thinking is to clinical practice.The third part is the clinical observation on the effect of prescription for QingHuoTongLuo and Xiaoyao Powder combined with TACE treatment of advanced liver cancer under the application of xiang thinking.The liver function and AFP after treatment in the treatment group as well as the control group were both significantly improved than those before treatment,and the differences were statistically significant(P<0.01);between groups,there was a difference in liver function and AFP after treatment between the two groups(P<0.05),the liver function and AFP of the treatment group improved more significantly.The incidence of adverse events such as abdominal pain,nausea,vomiting,poor appetite,fatigue,and bitter taste in the mouth in the treatment group were significantly lower than those in the control group(P<0.05).The improvement of quality of life and TCM symptoms in the treatment group were better than those in the control group(P<0.05).This clinical study shows that QingHuoTongLuo and Xiaoyao Powder combined with TACE can improve the quality of life of patients with advanced liver cancer,reduce adverse events,improve TCM symptoms,which has a good clinical effect,and it has a positive effect on the treatment of patients with advanced liver cancer after TACE.ConclusionXiang thinking is the traditional thinking of Chinese culture and plays a pivotal role in the origin and development of Chinese medicine.Applicational of xiang thinking not only explains the formation of traditional Chinese medicine theory,but also has a good guiding significance to the modern development of traditional Chinese medicine.Xiang thinking is the important foundation of traditional Chinese medicine which keeps the characteristic of traditional Chinese medicine to develop.Understanding tumor angiogenesis and guiding treatment from the perspective of xiang thinking,will complement the advantages of traditional Chinese medicine and western medicine,and service to clinical work better. |
