Research On Traditional Chinese Medicine Assisted Differentiation And Treatment Of Advanced Lung Cancer Based On Multidimensional Paradigm

Object:1.Based on literature review,data mining of medical records,multidisciplinary team discussion on real world integrated Chinese and Western medicine,and multi-dimensional response,the value of TCM assisted differentiation in the treatment of middle-advanced non-small cell lung cancer was discussed.2.To provide phased treatment plan for the standardization and unification of clinical TCM assisted differentiation and treatment in the western medicine treatment of middle and advanced lung cancer.3.Explore the construction of multi-dimensional paradigm of literature theoretical research,medical case data mining and multidisciplinary discussion of medical records.Method:1.Through the literature and theoretical research,the literature and theoretical research on the clinical treatment and mechanism of TCM for middle and advanced non-small cell lung cancer,the clinical treatment of western medicine for middle and advanced lung cancer and TCM assisted treatment are summarized.2.The data of all TCM assisted lung cancer clinical diagnosis and treatment cases in Nanjing Chest Hospital from January 2018 to December 2018 were collected and sorted out by MEDCASE Ver 5.2 TCM clinical Research assistance platform record mining system,and a number of integrated structured special disease case databases were established.Based on the research design of clinical informatics of TCM medical cases and the multi-dimensional mining processing method of big data,289cases of medical cases were statistically analyzed by using quantitative trend,association rules,cluster analysis and so on.According to the analysis results,the experience was summarized from the aspects of pathogenesis,treatment principle and prescription medication.3.A multidisciplinary team(MDT)with the participation of TRADITIONAL Chinese medicine was employed to make a detailed record of the clinical treatment,efficacy,related toxic and side effects,and the results of TCM assisted diagnosis and treatment for mid-and late-stage lung cancer in the real world.Result:1.Theoretical research on the treatment of advanced lung cancer by Traditional Chinese and Western medicine provides theoretical basis for clinical treatment.2.In this systematic study of clinical case data mining,121 cases were male,and the total number of male consultations was 168.Among them,94 were female,with a total of 121 consultations.Advanced lung adenocarcinoma incidence between the age of 50 to 80,the focus of the onset of age 60 to 79,clinical symptoms were obtained in 34,31 drug associated within the set,associated symptoms drug sets 33,symptoms of k-means clustering 10 class,symptoms of system clustering 7 classes,drug k-means clustering 10 classes,drug F10 level layered system 4 classes,Drugs F20 hierarchical system clustered into 4 categories,drugs F30 hierarchical system clustered into 4 categories,prescriptions k-means clustering into 13 categories.3.The participation of TCM in MDT discussion plays an indispensable role in the whole-course treatment of advanced lung cancer.Conclusion:1.Based on the multidimensional paradigm,it is found that TCM has advantages and characteristics in assisting the differentiation and treatment of advanced lung cancer,which has a wide clinical audience.2.The results of data mining showed that the symptoms of abnormal respiratory system functions such as cough,sputum and asthma were the main clinical manifestations,and drugs for reducing sputum,invigorating spleen,dampness,invigorating qi and nourishing Yin were commonly used in the adjuvant treatment of middle-advanced lung cancer with TRADITIONAL Chinese medicine.The symptoms of cough and phlegm in patients with advanced lung cancer are highly correlated.Erchen Decoction and its flavoring program are the main intervention programs in the adjuvant treatment of TRADITIONAL Chinese medicine.The focus of differentiation and treatment is mostly cough,phlegm,chest tightness and other lung symptoms.The core clinical pathogenesis of patients with advanced lung cancer is qi stagnation,phlegm and stasis,deficiency of qi and Yin,and heat injury to lung collaterals.There are 7 basic syndrome types in patients with advanced lung cancer,such as deficiency of liver and kidney,deficiency of spleen and kidney,deficiency of lung and liver stagnation,interaction of phlegm and blood stasis,deficiency of qi and Yin,and accumulation of phlegm and heat.TCM adjuvant therapy mainly focuses on six aspects:reducing phlegm and relieving cough,promoting qi and broadening the chest,supplementing qi and generating fluid,removing stasis and stopping bleeding,dispersing knot and reducing swelling,and strengthening spleen and stomach.The results of data mining and prescription tracing analysis indicate that the addition and reduction of TCM auxiliary treatment prescriptions for patients with advanced lung cancer mainly focus on traditional Chinese medicine prescriptions for regulating qi and reducing phlegm,nourishing qi and nourishing Yin,such as Foshou Powder,Erchen Decoction,Shashen Maodong Decoction,Sijunzi Decoction,pig ling Decoction,sweetened radix platycodon decoction,Qingjin Huatan pill and so on3.The MDT discussion on the role of TCM in the differentiation and treatment of advanced lung cancer has been further strengthened and supplemented.
