Objective:(1)Based on evidence-based medical research,meta-analysis and mesh meta-analysis system were used to evaluate the clinical efficacy of Lung Cancer(LC).Meta-analysis directly compared the efficacy of Traditional Chinese medicine compound combined with platinum chemotherapy compared with platinum chemotherapy alone in the treatment of LC,and mesh meta-analysis indirectly compared the efficacy differences of different traditional Chinese medicine compound.(2)Using data mining research ideas,mining and analyzing the medication rules of TCM compound therapy for LC in the real world,providing effective reference for the study of TCM therapy for LC.(3)Based on the research concept of network pharmacology method,explore the material basis and potential mechanism of TCM compound core drug combination in the treatment of LC at the micro level.Methods:(1)Evidence-based medicineAccording to the research objectives,the relevant retrieval strategies were developed in the Chinese National Knowledge Network(CNKI),Chinese Biomedical Literature Database(CBM),WANGFANG,VIP,The experimental group combined with Platinum based chemotherapy(PBC)was searched in Pub Med,Embase,web-of-sci,Cochrane and Web-of Sci databases.PBC alone was used to treat LC as the control group in the Randomized controlled trial(RCT)related clinical research literature,the corresponding data were extracted from the literature,Rev Man5.3 and Stata16.0 software were used for statistical analysis of the data.(2)Data miningCollected in jiangxi province,nanchang multiple levels of first-class hospital of traditional Chinese medicine compound in the treatment of LC prescription,using the inheritance of traditional Chinese medicine auxiliary platform to collected prescription drug taste for high frequency to the core,association rule,drug dosage,drug combination and drug clustering analysis,excavation and research of traditional Chinese medicine compound medicine for the treatment of LC.(3)Network pharmacology methodBased on the above studies,the core TCM for LC treatment was obtained,and the core TCM was taken as the main research object.Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform TCMSP)searched and screened The core TCM compounds and their corresponding targets in Gene Cards:The Human Gene Database,Gene Cards),Comparative Genomics Database(CTD),search and screen the LC disease targets,and the intersection of core TCM targets and LC disease targets is the action target of TCM treatment of LC.After using String platform to analyze the protein-protein Interaction(PPI)of the target,TSV files with the analysis results were imported into Cytoscape3.9.0 for topological analysis to obtain the core targets of LC therapy.Finally,Autodock was used for molecular docking of important compounds and key targets,and David platform analysis was used for gene ontology.GO)and the Kyoto Encyclopedia of Genes and Genomes,KEGG.Results:(1)Evidence-based medicine method was used to study the efficacy of Traditional Chinese medicine compound combined with Platinum based chemotherapy(PBC)to compare the efficacy of PBC alone.A total of 30 clinical studies were included,including 2228 patients,1116 patients in the observation group and 1112 patients in the control group.The results of meta-analysis showed that TCM compound combined with PBC was superior to PBC alone in improving clinical efficacy,T lymphocytokine level and reducing adverse reactions(GASTROINTESTINAL adverse reactions)(P < 0.05).The results of reticulated meta-analysis showed that nemorosa Nemorosa decoction could significantly improve the clinical efficacy compared with other traditional Chinese medicines.Traditional Chinese medicine combined with PBC can significantly reduce gastrointestinal adverse reactions,which is better than traditional Chinese medicine draba nemorosa var.leiocarpa Decoction,Morosa nemorosa var.leiocarpa decoction,Morosa nemorosa var.leiocarpa decoction,morosa nemorosa var.leiocarpa decoction,morosa nemorosa var.leiocarpa decoction.Compared with simple PBC,Yiqi Yangjing Decoction,Draba Nemorosa Decoction can significantly improve CD3+ level,among which buzhong Yiqi Decoction has better efficacy than Yiqi Yangjing decoction,Traditional Chinese Medicine decoction can significantly improve CD4+ level compared with simple PBC.In summary,draba Nemorosa var.leiocarpa decoction has significant curative effect in 5 indexes,which can be regarded as the optimal TRADITIONAL Chinese medicine compound for LC treatment.(2)Use data mining methods to explore the medication rules and core Chinese medicines for LC in the real world.A total of 407 effective real world prescriptions were obtained from clinical studies,involving 324 traditional Chinese medicines.The highest frequency of drug use was glycyrrhiza glycyrrhiza,Poria,Atractylodes atractylodes,Tangerine peel,Pinellia pinellia,Astragalus membranaceus,Codonopsis pilosula,Thunberg Fritillary bulb,Hedyotis willd and Coix seed.In the diagnosis of TCM syndromes of LC,phlegm-stasis syndrome was combined with syndrome(45.99%),the main treatment methods were dissipating phlegm-knot,detoxifying and anticancer drugs(28.99%,29.25%),and the main TCM drugs were deficiency tonic drugs(32.69%).The statistical results of taste and sex were temperature(41.39%).Bitter taste(45.5%),lung meridian(25.82%);Association rule showed that fuling-Pinellia,Poria cocos,Pinellia-Atractylodes atractylodes,Poria cocos,Thunberg fritillaria-Pinellia-Atractylodes atractylodes were the combinations with the highest support degree in the same group.The core drugs in the complex network were Codonopsis pilosula,Poria cocos,Atractylodes atractylodes,Tangerine peel,Fritillaria thunbergii,Astragalus membranaceus and glycyrrhiza.Based on the above research results,the core Chinese medicines were Atractylodes macrocephala,Poria cocus,Pinellia ternata and Fritillaria thunbergii.(3)Network pharmacology and molecular docking methods were used to study the mechanism of action and material basis of core TCM therapy for LC.A total of 89 targets were obtained in this study.The main effective components of the core Traditional Chinese medicine were 12-li photoacyl-8-trans atractylodes triol,14-acetyl coriolis photoacyl-8-trans atractylodes triol,β-sitosterol,etc.The important targets were AKT1,EGFR,TP53,etc.The enrichment results of GO molecular function showed that it may be related to the transcription regulation of RNA polymerase II,the reaction of cells to chemical pressure,and the reaction of metal ions,etc.KEGG pathway enrichment analysis results showed that the key pathways were enriched in tumor necrosis factor signaling pathway,platinum resistance pathway,IL-17 signaling pathway,etc.The docking energy between key targets and important compounds is less than 0k J/mol,and the docking conformation is stable.Conclusion:(1)traditional Chinese medicine compound joint PBC is in pure PBC can obviously improve the treatment process for patients with LC adverse reaction,improve curative effect,that has the potential superiority in the adjuvant therapy of traditional Chinese medicine LC and morel draba nemorosa soup for have been incorporated into compound optimal curative effect of compound Chinese medicine,traditional Chinese medicine for treatment of LC clinical practice to provide certain reference.(2)the data mining research shows that the real world of LC in traditional Chinese medicine(TCM)results,bitter in taste and warm in property,flat,cold,tonify deficiency is given priority to,in view of the LC pathogenesis and vital qi deficiency,blood stasis,phlegm and pathogenic poison into,tonifying qi and Yang,heat-clearing and detoxifying purging fire method to achieve the therapeutic effect of compound preparation of traditional Chinese medicines in the treatment of LC,atractylodes,poria cocos,pinellia,zhejiang fritillary is particularly important,It can be used as the core traditional Chinese medicine in treating LC.(3)The results of network pharmacology showed that the core TCM therapy of LC mainly achieved the effect of lung cancer treatment through multi-component and multi-target regulation of tumor necrosis factor signaling pathway,platinum-resistance pathway,IL-17 signaling pathway,etc.Its modern pharmacological effects mainly include anti-cancer cell proliferation,antibacterial,immunosuppression,etc. |
