Study On Risk Factors And Quality Of Life Of Primary Open Angle Glaucoma

To explore the risk factors of primary open-angle glaucoma (POAG) in women and evaluate the quality of life and influence factors in primary open-angle glaucoma patients. The purpose is to provide a reference basis for how to improve the quality of life in primary open-angle glaucoma patients and how to evaluate the treatment and the nursing effect in primary open-angle glaucoma patients comprehensively.The differences were explored between two groups by case-control study. The data of groups was based on hospital records from2009to2010which composed of128female patients with POAG and142female controls without POAG. The risk factors such as family history, hypertension, diabetes, age of menarche, age of menopause, parity, oral contraceptive (OC) use, hormone therapy were studied by logistic regression analysis.The convenience sampling survey was used on primary open-angle glaucoma patients of ophthalmic clinic in Huashan hospital of Fudan University in2010-2011and186cases were extracted. We investigated the quality of life and influence factors in cases with questionnaire and clinical examination including the scale of life quality for diseases with visual impairment and examination for visual function and questionnaire for influence factors.In logistic regression analysis, there were significant differences between two groups at family history (OR=48.05,4.63-410.90), hypertension (OR=6.51,2.76-15.31), age of menarche (OR=1.42,1.04-1.95), age of menopause (OR=0.62,0.39-0.99), hormone therapy (OR=0.26,0.07-0.96)The survey of quality of life in primary open-angle glaucoma patients showed that the average total score(YT) was127.06±40.67and the psychological dimension score(YP) was lowest20.37±11.85. In logistic regression analysis, the quality of life were significantly correlated with binocular visual field(regression coefficient β=-20.791, t=-7.010, P<0.001) and comprehensive integrated vision(regression coefficientβ=12.193, t=5.913, P<0.001). the less of binocular visual field defects and the higher comprehensive integrated vision the patients got, the higher of the total score and its four domains in the scale of life quality. The score of patients having adverse drug reaction was lower than others(regression coefficientβ=-13.469, t=-3.425, P=0.001). The score of older patients was higher than younger’s(regression coefficient β=0.579, t=4.089, P<0.001) especially in societal dimension and psychological dimension. The score of patients with higher education level was higher than that of lower ones(regression coefficient β=6.636, t=3.081, P=0.002)Family history and hypertension were important risk factors of POAG in women. Female hormones was a possible role for protection in the POAG.The life quality of POAG patients was generally decreased, with the poorest score of psychological dimension (YP). We need to pay attention to the life quality of patients with POAG, especially give necessary psychological counseling. Less of binocular visual field defects and the higher comprehensive integrated vision the patients got, the better life quality he would have. Adverse reactions of glaucoma drugs could affect the life quality of the patients. We should consider the drug efficacy in treatment, and the adverse drug reaction should also be noted. Life quality of older POAG patients was better than that of younger ones. We should pay attention to the young patients on psychological counseling. Life quality of POAG patients with higher education level was better than that of lower ones.
