Objective:Acne vulgaris is common in adolescents. Commonly involved sites are the face, chest, and upper back, Acne vulgaris is a chronic inflammatory skin disease that develops around the pilosebaceous apparatus and manifests as open and closed comedones as well as inflammatory papules, pustules, and nodules. Inflammatory acne can result in potentially disfiguring scarring and post-inflammatory hyperpigmentation, causing a serious physiological and psychological impact on patients. Acne pathogenesis is a multifactorial process in which Propionibacterium acnes proliferation infection and innate immunity play critical roles in pilosebaceous inflammation and abnormal hyperkeratinization. A large number of secretion of sebum production, alterations in sebum composition, and increased sensitivity to androgens, also play roles in the inflammatory process.In view of the acne pathogenesis, this study adopted the red and blue light combining Tanshinone capsules and Adapalene gel in treatment of acne vulgaris, Give comprehensive evaluation of the curative efficacy and security. Analyze the influence factors of the acne pathogenesis and explore its therapeutic mechanism.Methods:Select 180 appropriate patients with Acne vulgaris as the object of our study and group them randomly into three groups:red and blue light group (56cases), treat with red and blue light exposure, drug group (48 cases), treat with Tanshinone capsules and Adapalene gel, united group (76 cases),treat with red and blue light exposure, oral Tanshinone capsules and external use of Adapalene gel. We observed the curative efficacy about 4 weeks, observe and record the change of the skin lesion before and after the treatment, gather the image data, assess the adverse reaction during the process of the treatment weekly. At the same time we investigated the general situation of the patients with a clinical questionnaire including sex, age, course of disease, clinical classification, place of residence, occupation, and the influence factors of the acne’s incidence such as the environmental factors, diet factors, living habits, emotional state, physiology factors, genetic history and so on.Results:1. Red and blue light group had 54 cases (96.43%); drug group 46 cases (95.83%); united group 75 cases (98.68%), accept the whole course of treatment, the three groups completed a total of 175 cases,97.22% completion rate.2. The patients of the three groups had no statistically significant (P> 0.05) in age, sex, course of disease, clinical classification, place of residence, occupation and the score of lesions before treatment and other aspects, there were comparability.3. The incidence of acne were closely associated with the environmental factors, diets factors, living habits, mental state and physiological factors.4. The score of GAGS:the patients’score of GAGS of the three groups had no statistical difference (P> 0.05) before treatment within group comparison; and had significant statistical difference (P< 0.01) before and after treatment within group comparison; the red and blue group and the drug group had no statistical difference (P> 0.05) in the score of GAGS’comparison between groups after the end of treatment; the red and blue group and the united group had significant statistical difference (P<0.05) in the score of GAGS’comparison between groups; the drug group and the united group had no statistical difference (P> 0.05) in the score of GAGS’comparison between groups.5. The judgment of the curative efficacy:the total efficiency rate after the end of the treatment were tha red and blue light group 53.70%, drug group 58.70%, united group 82.67%. The red and blue light group and drug group had no statistical difference (P> 0.05) in the comparison between groups; The red and blue light group and united group had significant statistical difference (P< 0.01) in the comparison between groups; the drug group and the united group had statistical difference (P<0.05) in the comparison between groups.6. Adverse reaction:red and blue light group had 4 cases (7.41%), drug group had 7 cases (15.22%), united group had 10 cases (13.33%), there were no statistical differences (P> 0.05) in the comparison, all the patients in the three kinds of treatment had not discontinue treatment for the reason of serious side effects, they didn’t have been special treated and all relieved by themselves after the occurrence of adverse reactions.Conclusions:The therapy of red and blue light combining of Tanshinone and Adapalene has obvious synergistic effect, definite curative effect and high safety, and is worthy of applying and promoting on clinical treatment. Acne is a disease caused by a variety of factors and is moderate associated with the change of the environment, diet, living habits and so on, therefore, during the course of treatment, we should strengthen the related guidance.
