Background and PurposeUsually , the cholecystopathy patients with cholecystitis or cholelithiasis and post-cholecystectomy patients will suffer from upset of superior belly distension , belching , pain , nausea and so on . Some investigations revealed that these patients had abnormal motility , electrical rhythm of gastrointestine and the level of blood gastrointestinal hormone . Duodenogastric reflux is common clinical phenomenon , always existing the background of abnormal gastrointestinal motility. In order to comprehend gastrointestinal motility of post-cholecystectomy patients , we studied the patients' body surface electrical rhythm , gastrointestinal hormone and duodenogastric reflux .Now report as follows:Objects and Methods1. Objects 1.1 Cholelithiasis Group : Twenty (20) cholelithiasis patients whowere carried out cholecystectomy under larparoscope between September 1999 and January 2002 . They have no other organic origin disease (s) . 9 male , 11 female , ages range from 27-62 (mean 49.4?1.4) .1.2 Normal Control Group : Twenty patients who were examined to be healthy . 8 male , 12 female , ages range from 24 to 64 (mean 42.8?3.3).2. Checking Parameters2.1 Electrogastrogram Index : Three CPM in fasting and postprandial period, Bradygastria in fasting and postprandial period , Tachygastria in fasting and postprandial period , PDF in fasting and postprandial period , the ratio of PDP.2.2 Gastrointestinal Hormone Parameters : Motilin , Gastrin , glucagon, neurotensin.2.3 Duodenogastric reflux Parameters : DGR frequency (10 min ) , DGR strength , reflux index , contraction frequency of gastric antrum , contraction amplitude of gastric antrum , gastrodynamic index , emptying rate .2.4Bilitec 2000 Parameters : Alkali reflux times , long reflux times , the maximum time of alkli reflux , the total time of absorbance (abs)>0.14 , the time percentage of absorbance (abs)>0.14(%).3. Methods3.1 Electrogastrogram Examination : The examination was carriedout with Digtrapper EGG (Synectics Company , Sweden) , and8recording was performed in accordance with standard procedure . Test meal was 220 ml GUANGMING milk + 100 g ZHENWEI bread.3.2 Detection of gastrointestinal hormone : All blood samples are collected in fasting on morning. Gastrin : Taking 2 ml blood into a test tube , centrifuging at 4癈 , and the serum is taken for detection . Motlin , glucagon and neurotensin : Taking 6 ml blood into a tube containing 90 ul 10% EDTA and 80 ul aprotinin , mixing well and centrifuging at 4癈 , and then the plasma is taken for detection .3.3 Duodenogastric reflux (DGR) : The examination was carried out with Doppler Ultrasound . 7 cases patients with dyspepsia symptom of superior belly distension , belching , pain and so on were selected randomy to be examined with gastroscope and Bilitec 2000 in 10 days . All of them are female , ages range from 27 to 65 (mean 50.6?4.1).Patients were examined 3 days before operation and 10 to 15 days later after operation .3.424h monitor of bilirubin in stomach : The examination was carried out with Synectics Bilitec 24-hour bilirubin monitor. After fasting for more than 6 hours , the detecting probe was placed 5-10cm under LES of the patients . During the recording period , the patients are not allowed to intake foods with absorption spectrum similar to that of bilirubin . Theresults are analyzed with the software installed in thecomputer. All people didn't permit to take drugs influencing results3 days before detection.4. Statistics Analysis: Statistics process was carried out with SSPS 10.0 Software , a mean value was expressed in X 盨 . The comparison of mean values between groups was performed with t test . The parameters were analyzed with linear correlation . The comparison of coincidence rates of bilirubin monitor between DGR and Bilitec was performed with Exact X2 test.Results1. Before operation , the patient's three CPM in fas... |
