| Hyperuricemia is the major bilchemical base of gout. Hyperuricemia is accompanied with by gout. They have the same essence. Gout describes a number of disorders in which crystas of monosodium urate monohytate derived from hyperuricemic body fluid give rise to inflammatory arthritis, tenosynovitis ,bursitis,or cellulitis tophaceous deposits, urolithiasis, and renal disease. It is valuable to lower the serum uric acid level by different methods in clinic treatment and promote the development of new drug. Allopuriol and Uricosuric agents can be very effective in lowing the uric acid level, but the incidence of side-effect must be considered. So TCM method will be a new way in prevention and cure of hyperuricemia and renal impairment. Hyperricemia doesn't exist in Traditional Chinese Medicine term. It belongs to the categories of damp-heat, damp-toxic, Lijiefeng, gout. The renal impairment caused by hyperuricemia belongs to the categories of Guange, Shenlao, Nidu. TCM believes that gout can be caused by various pathogenic factors, such as improper diet, deficiency of spleen qi, damp-heat and so on . Damp-heat tends to obstruct the functional activities of qi and impairs spleen-stomach. There will be dysfunction of spleen in fluid transportation and transformation. Dampness is accompanied with heat, excessive heat can produce toxic. Dampness-heat-toxic damages the "kidney", and lead to inappropriate opening or closing the gate of water causing a backdown in the metabolism of water.Advisor Quzhuqiu has been dedicated in treatment of hyperuricemia and gout for many years. She believes that the location of this diseases is spleen and kidney, and the pathogenic factors include unhygienic diet, excessive drinking, also points out damp-heat attacting spleen-kidney is the pathogenesis. Advisor Qu uses the methods of clearing dampness heat, detoxifying, enriching spleen, nourishing kidney. The prescription Tongshenning derives from"Ermiaosan" . The ingredients of Tongshenning are Rhizoma Atractylodis, Cortex Phellodindri, Radis et Rhizoma, Rhei herba Leonuri, Radix Astragali, Herba hedyotis Diffusae, Polyporus. This treatment can not only strengthen body resistance with pathogenic factors repelled but also eliminate pathogenic factors without hurting the vital-qi. It can treat both the incidental and the fundamental at the same time .The emphasis of the formular's function is the concept of the Organism as a Whole and Diagnosis and Treatment Based on an overall Analysis of signs and Symptoms.In order to verify and explore the effect and the mechanism of tongshenning, we carried out the study of this project. Initially we succeeded in establishing the animal model of experimental hyperuricemia and renal impairment and made the base of next step. In formal test, high, medium, low dose are used on rat. We set up contrast, observed the common reaction, serum uric acid(UA), renal function(Cr, Bun),and the change of pathological histology of kidney. The results indicated Tongshenning can lower the level of uric acid, the medium dose show the best performance and its UA level is lower than the model group in the 14th and the 21th day (P<0.05). Tongshenning can improve the renal function of experimental rat, the level of Cr and Bun in medium dose group is quite near to the normal group (P>0.05). Tongshenning can repair the pathological impairment of kidney, further more, under observation of photoscope and electric scope, no abnormality was found in kidney. |
