Background: There is lack of an objective diagnosis standard in the sthenia and protration ZHENGs of stroke.Objective: To study the relationship among the sthenia and protration ZHENGs of hypertensive cerebral hemorrhage (HTCH), the changes of the plasma atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP), the hemorrhage volume, and the scores of nervous impairment, and then to provide the objective and quantitative diagnosis standard for the sthenia and protration ZHENG in HCTH.Materials and methods: Fifty-eight cases of HCTH who accorded with the diagnosis of HTCH and the sthenia and protration ZHENGs of stroke were observed. Among them, there were 46 sthenia ZHENG, 12 protration ZHENG; 36 male and 22 female, and the average ages were 63.94 11.77 (45-83 years old). 30 normal subjects were observed as control, there were 16 male, 14 female, and the average ages were 60.39 8.94 (51-72 years old). Another 30 cases of Channel attacking group were observed as control too, there were 18 male, 12 female, and the average age was 61.28 10.37 (49-81 years old). The standard of nervous impairment of acute cerebral accident was used to score, the DUOTIAN formula to calculate the hemorrhage volume, the RIA to determine the content of plasma ANP, and the EL1SA to determine the plasma BNP.Results:1 . The plasma ANP and BNP of the channel attacking and organ attacking groups significantly increased in comparison with the normal control groups (P<0.01).2. The plasma ANP of the protration ZHENGs was significantly higher than that of sthenia ZHENGs (P<0.01), while the plasma BNP was not obviously different between the two (P>0.05); The ANP of protration ZHENG was significantly higher than that of YANG sthenia ZHENG (P><0.05), while plasma BNP in the former lower than that in the latter (P<0.05); The plasma ANP and BNP of protration ZHENG were higher than that of YIN sthenia(P<0.01) ; The plasma ANP and BNP of YANG sthenia were higher than that of 7/W sthenia(P<0.01).3. The hemorrhage volume was significantly different between the sthenia and protration ZHENGs in HTCH (P<0.01). The hemorrhage volume of protration was the first, YANG sthenia second, YIN sthenia last.4. The scores of clinical nervous impairment were significant different between the sthenia and protration ZHENGin HTCH (P<0.01). The scores of protration was the first, YANG sthenia second, YIN sthenia last.5. The plasma ANP and BNP in HTCH showed obvious positive correlation to the scores of nervous impairment, hemorrhage volume (P<0.01).6. The sthenia and protration ZHENG in HTCH can be discriminated by levels of ANP and BNP in plasma and hemorrhage volume,93.3% of original grouped cases correctly classified; the sthenia and protration ZHENG can be discriminated by levels of ANP and BNP ,91.8% of original grouped cases correctly classified.Conclusion:1. The plasma ANP and BNP were related to HTCH and could reflect its degree of brain impairment.2. Both the sthenia and protration ZHENGs in HTCH could reflect the degree of brain impairment.3. The diagnosis of sthenia and protration ZHENGs in HTCH can be standardized and quantified based on the plasma ANP and BNP, the hemorrhage volume and the scores of nervous impairment.
