The research is one part of "evaluation of an experimental integrated community health care system for urban china" on effect for hypertension patients who have their contracts with community health center (CHC). By analyzing the rate of awareness, treatment controlling of hypertension, the popular rate of health acknowledge and the rate of accepting health behavior, a conclusion was made that health level of hypertension patients has rise rapidly after intervention. By assessing the level and the effect of implementation of the CHC model, the legitimacy and sustainability were evaluated. The aim of research is seeking for a effective model on community intervention for chronic disease.The proposed intervention has for parts: creation of a new clinical model, improving coordination of care, restructuring financial management and integration of health information systems. For hypertension patients in community, the model emphasizes contract-based management, comprehensive treatment and prevention, and information systems. The treatment is more system and standard than before, which includes assessment in absolute risks for hypertension patients. According to different risk levels (normal, light, middle and serious) there are different treatment plans for hypertension. The program requires general practitioner (GP) following-up patients regularly. Patients have taken both on drug and on no drug.After intervention, the rates of both awareness and control were increased and the rate of treatment by drug was as the same high level as that before.. The rates of knowledge for high blood pressure and exercitation were raised, for some extent. The average level of Blood pressure decrease significantly after intervention especially for those people whose BP were abnormal before the project, which were drop by 10.30mmHg(SBP)and 4.77mmHg (DBF) in Zhongguancun CHC, 14.06mmHg(SBP)and 5. 97mmHg (DBF) in Yulin. The control rate of hypertension raised from 38.39% to 64.29% in Yulin CHC.and from 59.62%to 79.40% in Zhongguancun CHC. The questionnaire survey on knowledge, attitude and behavior indicated that the awareness rate of the rule ofhypertension was increased and it was higher in case than in control after preventision. The awareness of risk factors of high blood pressure(fat, salt, drink, nervous, heredity) was highly in case than that in control in Yulin. Suggests in behavior achieved from GP in case was more than that in control. The rate of hypertension patients taking doctor's instructions was highly in case than in control. More and more patients accepted and took good action such as nutrition, salt, exercise, smoke, drink, mentality and or so. It was concluded that after intervention hypertension absolute risk level was drop by compared with that in before. It indicated that the plan integrated with synthetic treatment and health education was available and feasible.Project aims have been achieving step by step. The project has increased accessibility of health care through home care, improved continuity through roster family physicians, improved comprehensiveness with a holistic approach to patient care, improved technical quality of care through training family physician constantly. Patient and staff satisfaction is high, and local and central authorities support the intervention. The model has been copied independently by other clinics in the surrounding regions, which suggests increased legitimacy. Increased effectiveness of hypertension and relate disease and profitability indicate that the model is sustainable.Therefore the new model which was comprehensive in treatment and prevention for hypertension patients can be able to control and reduce their BP level, to increase the rate of blood pressure, to improve health and life-quality. It was promising to achieve the essence aim.
