Objective To analyse and investigate the relationship between early cognitive disorder and hemiplegia or aphasia in patients with acute stroke,also to observe the recovery course and prognosis of cognitive disorder.Methods 50 patients with certain acute stroke,use LOTCA to evaluate their cognitive function, the evaluate cycle is half month,1 month,2 months and 3 months since the onset,amount to 4 times.And use WAB to divide the patients into two groups,one is aphasia group including 30 patients,the other is nonaphasia group including 20 patients Also we use FMA to evaluate their kinesis function,the evaluation of aphasia and kinesis function is given within half month. With SPSS statistic software package to calculate the data.Results The total score of cognitive function is significantly related to the FMA score of hemiplegia patients with subcortical lesion(r=0.523,P<0.01).Among which orientation,perception disorder and optomotor tissue score are significantly related to the FMA score of hemiplegia patients with subcortical lesion,which are positive correlation. (r=0.495,0.515,0.583,P<0.01) ,there is no significant relationship between the score of thought operation and the score of FMA(r=0.306,P>0.05), there is no significant relationship in LOTCA score between right hemiplegic group and left hemiplegic group(P>0.05);there is significant relationship in LOTCA score between aphasic group amd nonaphasic group(P<0.05) .The cognitive function scores evaluation suggested that the ability of orientation,optomotor tissue,and thought operation scores gradually improved after lst,2nd and 3rd month,there is significant differences between the score of harf month and lst,2nd,3rd month(P<0.05).But there is no significant difference between 1st and 2nd month scores,together between 2nd and 3rd month score(P>0.05). there is nosignificant difference between the perception score of half month and 1 st,2nd and 3rd month,2nd and 3rd month(P<0.05), but there is significant differences between the perception score of half month and 2rd,3rd month, 1st and 3rd month(P>0.05). Conclusion There is significant positive relationship between the cognitive impairment and the hemiplegia of subcortical lesion of acute stroke patients .The heavier cognitive impairment the heavier hemiplegic in subcortical lesion patients.So the cognitive impairment shoud be evaluated and treated in the patients with heavier hemiplegia of subcortical lesion. The lesions of left or right hemisphere both can cause different degree or different types cognitive functions.In the patients with acute stroke,orientation,optomotor tissue,and thought operation function recover rapidly within 1 month, and recover slowly 1 month later. Imperception recover rapidly within 2 months.Cognitive function in stroke patients with aphasia is significantly reduced.Thus,it is very essential and effective to evaluate and treat the cognitive function in the patients with aphasia. |
