| Objectives To discover the distributive characteristic of symptoms about osteoarthritis of knee and to summarize the differentiation of these syndromesMethods Select Six TCM hospitals in Hunan provience at randoms,ad-opt clinical epidemiologic questionary suyvey methods to find out the main symptoms, apply PROXSCAL to analyze pathogenesis,depend on cluster and logistic regression to induce the type of syndrome and signs for classification of syndromes.Result At last collect 624 valuable questionary.This disease is 2.54 times more common in woman.Average age of the sample is 57 ?6.07,ov-erweight is more than 70%,both knee are infected is more common as well as light and moderate knee joint pai:i,distend and cold type pain are more often,dynamical factors ,such as going-up-and-down-stairs and stepping,are primary inducement. About half of the "case has ankylosis in the morning as well as not able to went down perfectly. 22.4% patient totter.Ache points are often located by patella,less than 10% case ha -ve abnormality,thigh atrophy is prominent,patella push is more common. Kellgren and Lawrence's grade of X-ray is always from 1 to 3. The main sympotoms is knee joint pain,pain during going up and down stairs, pain in walking, From high to low the frequency of pathogenesis isinsufficent of kidney-YANG, blood stasis,cold-damp,difficiency of liver-YIN,damp-heat,weakness of spleen, sputum,wind. It is obvious that path o-genesis is divided into two groups. YIN is include insufficent of kidn eyYANG,weakness of spleen, cold damp,wind,sputum;YANG is included deficiency of both liver and kidney YIN,stasis,damp-heat. Differentiation of sympotom and signs for classification of syndroms is listed.Weakness of liver,spleen and kidney with blood stasis:dizziness,dim complexion, weakness of lumber region and knee joints,anorexia,spontaneous perspir-ation,greasy fur on the tongue,weak,deep and stringy pulse;Insufficient of kidney YANG and liver YIN with stask;:swelling of the knee joints, cold type pain,sensation of pricking,distending pain,pain during a change in the weather, ankylosis in the morning, coldness of the body with chills, sleeplessness and dreaming often,dryness and uneasy feeling of the eyes,dry mouth with bitter taste,cold and pain both lumber region and knee joints,constipation,polyuria at night,pa e or light red tongue,thready pulse; Deficiency of both kidney and liver YIN: ankylosis in the morn-ing,burning pain, dim eyelid, dryness and uneasy feeling of the eye,dys-phoria with feverish sensation of in the chest palm and soles,dry mouth with bitter taste,red tongue with ecchymosis,stringy and fast pulse;Insuf-ficiency of kidney YANG with blood stasis and cold-damp:swelling;col-dness of the body with chills,clear urine in larger amounts,pain exaggerated by catching cold,soreness of lumber region and knee joints,polyu-ria at night,weak and deep pulse.Conclusion The basic pathogenesis is weakness of kidney and blood st-asis. the type of syndromes includes weakness of liver and kidney:ach- ingpain,secret anguish,soreness of the lumber region and knee joints,dizziness and blurred vision,tinitus like cicada chirpping, polyuria at night, light tongue,white and thin fur,slim and fragile pulse;stagnation of blood stasis:distend or prick pain in the knee joints,immobility pain,aching at night,tenderness,dark complexion,tamishing facejight purple or ecchymo-sis tongue,white and thin fur,slim and deep or slim and slim or acerb-ite pulse;stagnation of cold damp:cold type pain,heaviness, exaggerated by catching cold, coldness of the body wiih chills,white,slipperry and moist fur,deep stringy pulse; weakness of spleen and kidney:secret anguish in the knee joints, dim complexion,dizziness,anorexia, spontaneous perspiration,greasy and white fur,deep weak pulse;defficiency of both kidney and liver YIN: aching pain,soreness of the lumber region and knee joints, dysphoria with feverish sensation of in the chest palm and s-oles,dry mouth with bitter taste, dim eyelid, dryness and uneasy feeling of the eye, red tongue,uncoate... |
