Objective To explore the diagnostic value of BAEP in the vertiginous patients who are different age, sex and type in order to provide evidences in clinical diagnosis, we analyze brainstem auditory evoked potential (BAEP) in all kinds of vertiginous patients and combined their detailed history and correlative laboratory investigations by reviewed.Methods 142 vertiginous patients who followed other systems or not were referred in our hospital between January 2001 and June 2003. Patients were divided three groups (A, B, C group) according their ages, then two sub-groups (A1 and A2, Bj and B2, C1 and C2 group) according their sex. In addition, patients were also divided isolated vertigo group and vertigo group accompanied other neurologic symptoms (X and Y group) according vertigo accompanied brainstem injury. All patients must be examined BAEP in one week. The peak latency(PL)and Amp of wave I , III , V, the interpeak latency (IPL) of wave I -III , III- V, I - V ,the interear latency difference (ILD) of two ear , the ratio of V/ I (Amp)and III-V/ I -III (IPL) were observed. Meanwhile we also acquired detailed knowledge of the history and correlative laboratory investigations.Results (D Of the 142 cases, BAEP studies disclosed abnormalities in 66 (46.5%). The abnormality rate was distinct in the different age ,and the rate increased following by growing age (P<0.05 ) . The abnormality of A, B, C group was 4 (25.0%) , 26 (40.6%) and 36 (58.1%), respectively. There were no different when different sex were compared with each other in all ages (P>0.05) .(3) Of the 66 cases with isolated vertigo, there were 21 abnormal cases (31.2%) . But 76 cases who were vertigo accompanied other neurologic system ,45 cases ( 59.2% ) were abnormality. There was statistical difference in each other (P<0.05 ) . (4) Brainstem type was the most type in all abnormal groups, but inner-ear type and complex type were less. The abnormal type was no difference in kinds groups (P>0.05) .(D 53 of the 142 patients underwent TCD, and the abnormality were 32 cases (60.4%) . The abnormal ratio was no difference in A , B, C group and X, Y group (P>0.05) .Conclusion Long-term and short-term vertebrobasilar ischemia is the most reason of abnormal BAEP in patients with vertigo, but inflammation and demyelinating disease and so on are not ignored. With the growing of age, abnormality rate of BAEP increases. Abnormality rate of BAEP is lower in isolated vertiginous patients than that of brainstem injury. Isolated vertiginous patients with abnormal BAEP should be paid attention to, then normal BAEP is caused by peripheral vestibular disorders. BAEPIVcan do topical analysis for injury location of vertiginous patients, and the different injury range may be implied by unilateral and/or bilateral abnormality. Combined the history, BAEP is helpful to diagnose for patients with vertigo. |
