BackgroundThe ideology and strategy have been made a rapid progress in the past 30 yearsthrough the endeavoring of many clinician in the world, but SAP still termed as a poor prognosis emergency abdominal disease with a high mortality (20~30%). So the research on the SAP is still a hotspot in clinical surgery today. Establishment of an ideal SAP animal model is the precondition in exploring the mechanism of SAP. An ideal SAP model should be providing with classical morphologic changes, which has similar pathophysiological characters with the human SAP. The SAP model also should have traits in stable, repeatable, cheap and simple in the procedures. Scholars established many SAP animal models for their own purpose and the research orientations. There are about more than 7 types of SAP animal model recorded in the literature, and each model has its own advantages and indications. So there still has no an ideal model can be regarded as satisfied model suited for research in universal. The SAP rat model induced by 5% sodium deoxycholate retrogradely common biliopancreatic ducts injection via duodenum puncturing was first reported by Aho HJ, and it was regarded the most classical SAP animal model which was widely adopted by SAP researcher in the world. Compared with other SAP animal model, pathophysiological changes induced by Aho's method mostly have similar characters with human SAP process, especially in the pancreatic microcirculation disorder and hemorrhagic necrosis pathological changes in the pancreas. But we find it still has many disadvantages through our practice, mostly in excessive high mortality and animal blood lose caused by puncturing the common biliopancreatic duct. Unstable and excessive severe SAP model induced by Aho's method make the clinical drugsinterfere experiment more insensitive, especially in Chinese traditional medicine research in SAP. So we made some techniques improvement in the establishment of SAP model based on the Aho's method. In practice, we introduced 3.5% sodium deoxycholate in the model induction, and it was injected by micro-regulated pump via papilla duodeni through cavitae epiduralis anaesthesia channel catheter. The SAP animal model was successfully induced after these techniques improvement and benefits was achieved in our experiment. Material and methodsHealthy male Sprague-Dawley rats were adopted in this study (average weight 250~300g). SAP model was induced by modified Aho's method. 3.5% sodium deoxycholate was injected retrogradely through common biliopancreatic ducts via papilla duodeni by cavitae epiduralis anaesthesia channel catheter. The rats were randomly assigned to SAP model group and shame operation group. The mortality and ascites volume were calculated in each group. The serum amylase activity at 3h, 6h and 12h were measured respectively.The tissue sample of pancreas and extra-pancreas organs (lung, liver, kidney, et al.) were harvested at 3, 6,12hour in each group. Organs tissues were prepared by paraffin fixation for the pathological morphological observation, and pathological severity degree of pancreas were appraised by Schmidt score system under the conventional light microscopy. Results1. The mortality of SAP model group at 3, 6, 12hours were 0% ( 0/15 ) ,13.33% (2/15 ) and 33.33% ( 5/15 ), the serum amylase activity were 5522.47±1252.80, 6229.92±1212.45 and 7283.50±l 884.71 (u/L) , and the ascites volumes were 6.24±1.88,9.56±1.87 and 8.63±1.64( ml) respectively. The severity of pancreas morphological changes appraised by the Schmidt's score system at 3, 6, 12hours were 3.80±0.75, 4.54±1.20 and 5.61±1.23. The indexes in the shame operationgroup were nearly normal, and it indicates open abdominal operation has noinfluence on the SAP model induction. 2. The SAP rat model was successfully induced by our method. The pancreas ofSAP rat has classical morphological changes in hemorrhagic necrosis andinflammatory cells infiltration under the microscopy observation.
