Objective To study the expression of heterogeneous nuclear ribonucleoprotein A2/Bi (HnRNP A2/B1) and HnRNP B1 in lung tissue and serum as well as sputum of patients with lung cancer and evaluate its sensitivity and specificity in the diagnosis of lung cancer. Method The expression of HnRNP B1 in lung tissue and sputum were detected by immunocytochemistry or immunohistochemistry method using specific monoclonal anti-HnRNP B1 antibody in 150 tissue samples and 70 sputum samples of patients with lung cancer controlled by 30 normal subjects; The expression of HnRNP A2/B1 in serum were detected by RT-PCR method in 60 patients with lung cancer using specific primers of HnRNP A2/B1 and HnRNP B1; The expression of HnRNP B1 in serum were detected by ELISA method in 44 patients with lung cancer using specific monoclonal anti-HnRNP B1 antibody.Results (1) Expression of HnRNP B1 in lung cancer tissues The immunohistochemistry results with 46 positives in 46 samples of squamous cell carcinoma and 45 positives in 51 samples of adenoma while 33 positives in 37 samples of small cell carcinoma and 14 positives in 16 samples of largecell carcinoma. The positives rate was totally 90%. (2) Expression of HnRNP A2/Bi mRNA in lung cancer cell lines RT-PCR indicated the presence of HnRNP A2/B1 mRNA were detected in the lung adenocarcinoma cell lines SPC-A1 and Y-90, squamouse cell carcinoma cell line A431 and small carcinoma cell line NCI-H446. (3) Expression of HnRNP A2/B1 mRNA in peripheral blood samples RT-PCR results with 49 positives in lung cancer group and average quasi-quantitative ratio is 0.657 ±0.326 while 3 positives in control group and the ratio is 0.159 + 0.286, the express quantity before therapy were significant higher than that after (PO.01); The expression rate of HnRNP A2/Bi mRNA and HnRNP B] mRNA in serum are 73% and 63% respectively before radiology therapy while 56% and 50% after therapy, their expression rates before chemotherapy were 76% and 60% while 63% and 46% after chemotherapy. The expression rate is no significant difference between that before therapy and after therapy. (4) Expression of HnRNP Bi in peripheral blood samples ELISA results with 33 positives of 44patients with lung cancer and 1 positive of 20 control samples. The average OD value in lung cancer group(0.252 + 0.150) is significantly higher than in control group(0.022± 0.019) (PO.01). (5) Expression of HnRNP B, in sputum defoliated cells of patients with lung carcinoma Theimmunochemistry results with 50 positives of 70 lung cancer samples, with 23 positivess in 29 squamous cell carcinomas and 17 positivess in 26 lung adenoma, while 10 positivess in 15 small cell lung carcinoma . There are 33 positivess in 40 central lung cancers while 17 positivess 30 peripheral type lung cancers. (6) Values of HnRNP A2/Bi in diagnosis RT-PCR to HnRNP A2/B1 in serum gives a 81% sensitivity and 90% specificity. Serum ELISAto HnRNP Bi gives a 75% sensitivity and a 95% specificity. Sputum IHC toHnRNP Bi gives a 71% sensitivity and 90% specificity .Conclusion HnRNP A2/B1 and HnRNP BI overexpressed in lung cancertissues , lung cancer cell lines, peripheral blood and sputum defoliated cells,the examination of HnRNP A2/B1 and HnRNP Bj in serum and sputumsamples with RT-PCR, ELISA and IHC method are sensitive and specific indiagnosis of lung cancer.
